The transition coordinator identifies, coordinates or provides continuity of care and appropriate levels of care under the direct supervision of an RN for Hoosier Care Connect members. The transition coordinator is responsible for coordinating care management and utilization management information as members move into and out of the health plan. This role will be based in Indiana.
If you reside in Indiana, you will have the flexibility to work remotely* as you take on some tough challenges.
Primary Responsibilities:
Oversight of all member transitions in and out of the Hoosier Care Connect program and the Contractor’s enrollment
Ensures the transfer and receipt of all outstanding prior authorization decisions, utilization management data and clinical information such as disease management, care management and complex case management notes.
Establishes relationships with the MCE Transition Coordinators
Documents transition coordination activities in the member’s record in the care management platform, CommunityCare
Serves as a Liaison with the Department of Child Services, establishing relationships and ongoing communication to promptly resolve any issues identified
Aids in the transition of the Hoosier Care Connect wards and foster members
Participates in other care coordination duties as assigned

Required Qualifications:
1+ years of professional experience working with Indiana Medicaid
Experience working in a member outreach, managed care or care coordination setting
Resident of Indiana
Preferred Qualifications:
Experience working with wards and fosters or the Department of Child Services*All employees working remotely will be required to adhere to UnitedHealth Group’s Telecommuter Policy
