Position Summary
The Team Leader of Home Based Crisis Intervention (HBCI) program will advance the Agency's Mission and Strategic Plan by providing short-term, home-based crisis intervention services to youth and their families who are at risk of institutional level of care, and who require intensive interventions in order to adequately support the child and family’s complex needs. This position will be responsible for the overall management of the program, to include but not limited to: staff matters, client referral, client treatment, financial viability, reporting to grant funders, marketing of program, program manual, program systems, etc. This position will be responsible for the delivery of crisis intervention services compliant with the Homebuilder Model. This staff may participate, as needed, in Single Point of Access meetings and quarterly subcommittee meetings of the Community Services Board and will support data collection for program monitoring and outcome reports as required.
Essential Duties and Responsibilitiesinclude the following. Other duties may be assigned. Employees are expected to be awake and alert while working.
Provides overall supervision of the team and is responsible for allocation of staff time
Ensures all team members are kept informed of issues involving participants and their families
Monitors caseload size and coordinates program intakes and discharges
Provides training and supervision for the staff
Makes rotating call schedule for after-hours / weekend coverage by team members
Educates the community about HBCI at Northern Rivers; philosophy, program outcomes, theoretical background etc.
Develops and maintains collaborative relationships with other agencies, schools, colleges, institutions, hospitals, primary care providers, and local government agencies to assist in meeting the needs of this population so they can achieve and maintain their independence
Participates in all required training as directed by funding source and ensures that staff are also in compliance with all funding source training
Serves as the primary clinician for a subset of program participants. In this role the Team Leader will:
Serve as the point person for the participant and their family
Conduct needs assessments
Engage in safety planning and wellness management planning with participants
Connect participants and families with the services and support they need
Provide psychoeducation regarding mental health diagnosis and treatment
Develop and modify treatment using a shared decision-making process with clients
Respond to on-call needs of families in crisis and deliver crisis intervention services in home-based environment with flexible scheduling cognizant of family needs, with 24/7 availability nights, weekends and holidays.
Promote safety of the child, family, and community in the delivery of crisis intervention services.
Develop a positive working relationship with families so as to enhance engagement and motivation.
Conduct comprehensive assessment of family strengths, values, skills, problems, need and barriers to goal attainment.
Apply cognitive and behavioral principles and strategies to facilitate behavior change.
Use a variety of teaching methods to help family members acquire, maintain and generalize skills.
Collaborate and advocate with formal and information community resources and systems that impact family while teaching family members to advocate for themselves.
Prepare family for transition and service closure.
Participate in the Continuous Quality Improvement process and provide leadership in the collection of data and the formulation of recommendations and conclusions as assigned.

Qualifications

Education and/or Experience Requires a Master’s degree and at least three years of progressively responsible related experience. Experience in a home-based and/or crisis intervention services program. Candidates with training in the Homebuilder model of crisis intervention are preferred.
Certification/Licensure
Must be a licensed mental health professional. These include, but are not limited to: Licensed Psychologists, Licensed Clinical/Masters Social Workers, Licensed Marriage and Family Therapists, Licensed Mental Health Counselors, or Licensed Creative Arts Therapist. Must have a valid driver’s license.
