Job Summary:
The Supervisor of Community Outreach and Care Coordination (“Supervisor”) assists the Director of Community Outreach and Care Coordination in carrying out the administrative functions needed to operate the Children’s Health Home Care Management program, High Fidelity Wraparound Care Management program, and the Lansingburgh Connects School Based Mental Health Program. The Supervisor provides direct supervision to a team of 3-6 Health Home Care Managers and Family Engagement Specialists, and acts in the capacity of the Director of Community Outreach and Care Coordination in their absence.
Administratively, the Supervisor is responsible for outreaching to and enrolling youth into the program, as well as onboarding and training newly hired Children’s Health Home Care Managers and Family Engagement Specialists when necessary. The Supervisor may be required to carry a temporary caseload of enrolled Health Home and Lansingburgh Connects youth during periods of staffing shortages.
Duties & Responsibilities include:
Provides outreach to referrals to establish eligibility, and enroll them in the Children’s Health Home program when appropriate.
Assists with the training and onboarding new employees by shadowing, mentoring and coaching new hires and ensuring all agency and Department of Health (DOH) required trainings and requirements are completed.
Prepares monthly capacity reports for the lead health home and ensures staff comply with annual mandatory department of health training.
Prepares reports as needed for the Lansingburgh School District and New York State Office of Mental Health
Assists with review of Health Home case records to ensure compliance with all regulatory requirements as set forth the by DOH, the contracted Health Homes and the MCO’s.
Provide direct supervision to a team of 3 to 6 full-time Children’s Health Home Care Managers and Family Engagement Specialists.
Assists Director in planning for the growth and the strategic direction of the program.
Attends meetings with the Lead Health Homes, Lansingburgh School District, NYS Department of Health, NYS Office of Mental Health, local District SPOA’s and with other community stakeholders as required.
Participates in agency and Health Home Quality Assurance meetings.
Completes all mandated training as set forth by DOH, the contracted Health Homes and the MCO’s.
Assists with the distribution and collection of bi-annual satisfaction surveys.
Additional position-related duties and responsibilities as assigned by the Director of Health Home Care Management and the Director of the Community Outreach and Care Coordination as appropriate.
Position Specifications:
Bachelor’s degree in psychology, sociology, counseling, or other human service related field.
4-5 years of experience in human services, including child welfare, mental health, substance abuse and physical health. Less experience considered with an advanced degree.
Supervisory experience preferred but not required.
General knowledge of Computer applications: Microsoft Office Suite (Word, Excel), Google Business Workspace Suite (Gmail, Google Sheets, Docs, Meet, etc.), and the Internet.
This position does have regular and substantial unsupervised or unrestricted contact with individuals receiving services and is required by law to be fingerprinted for a criminal history record check under DOH (NYS Department of Health).
Local travel is required; Must have a valid NYS Driver’s License and have reliable transportation.
Must possess the ability to make sound, independent decisions when circumstances are warranted.
Ability to pass the test with a .80 or better to become a certified CANS assessor.
Ability to pass required pre-employment background checks, to include fingerprinting for DOH, OMH, & OCFS.

The employee frequently is required to stand, walk and sit. Must be able to sit for long periods of time.
