Job Summary
As the Nurse Practitioner for our Cardiothoracic program, inclusive of Transaortic Valve Repair (TAVR). In accordance with standardized procedures, performs physical examinations and treats common episodic and health care conditions with an emphasis on health maintenance and disease prevention through education and counseling. Works in collaboration with other health care team members, consulting and referring as appropriate, but retains responsibility for ongoing clinical management of the patient. The Nurse Practitioner duties will include but not be limited to: Interviews with patients and/or family, construct comprehensive medical histories, perform initial and periodic physical examinations, evaluate patient behavior and explore patient's interpretation of health needs. Directly performs or orders appropriate diagnostic studies and interpret finished results. Develops and implements long and short range treatment plans. Writes orders to increase, decrease, or change medication (subject to checking and medical consultation as indicated). Maintains required documentation and consults with physician on changes in the patient's condition. Counsels patients and families as to preventive care, medical problems, psychological problems, and use of prescribed treatment and drugs. The nurse practitioner will have specific privileges under the bylaws of Citrus Valley Health Partners Medical Staff Office. A copy of the practitioners privileges can be obtained from the Medical Staff Office. The practitioner will be evaluated by designated physicians taking care of identified patients, inclusive of but not limited to co-signing written orders, following up on treatment protocols and discharge summaries, as well as 80% of charts written by the nurse practitioner will be reviewed by an associate physician. As the Nurse Navigator for the Cardiothoracic Service Line to include the following duties: Clinical education/resource nurse working collaboratively in a patient care setting to assess and address the needs of patients throughout the cardiac care continuum. Conducts daily rounds, reviews patient plan of care for core measure compliance and provides patient education ensuring efficient and optimal care of Structural Heart service line patients. Identifies and refers discharge issues to attending physicians, nursing, Care Coordinators, or others as appropriate to maximize the patient`s experience and outcomes. Participates in oversight and data collection relative to registry compliance with the TVT registry, monitors clinical outcomes and identifies areas for performance improvement. Proactively helps plan and coordinate the efficient movement of the postoperative patient through the continuum of care, based on medical needs, functional ability and available resources. Develops and presents staff education programs based on assessed needs.
M inimum Education Requirement :
Possession of a current certificate of completion from a Nurse Practitioner program. Two or more years clinical nursing experience.
Minimum Experience Requirement :
Minimum of 5 years` experience in adult critical care and cardiovascular /cardiothoracic surgery. Must demonstrate effective communication skills, expert leadership capabilities, be detailed oriented and capable of multi-tasking and responding rapidly to changing clinical situations in inpatient and outpatient care arenas with high clinical acumen and strong crisis management skills. Highly motivated and self- directed, will demonstrate ability to function independently and exhibit high levels of independent problem solving skills and decision making capabilities. Excellent written and verbal communication skills required. Excellent customer service skills required.
Minimum License Requirement :
Current California RN license required. Nurse Practitioner furnishing license, DEA license, NPI number, ACLS and BLS.

$54.36 - $77.73
