Summary:
The Clinical Stroke Coordinator-Advanced Practice Provider (APP) is responsible for the oversight of the Stroke Program at Mount St. Mary's Hospital (MSMH). The Stroke Coordinator APP works in collaboration with the Comprehensive Stroke Center leadership, Medical Director of Stroke at MSMH and MSMH Nursing Administration to lead process improvement, meet core measures for stroke, lead certification processes, and ensure providers and nurses have met educational requirements. The Stroke Coordinator APP provides the leadership of the facility's stroke program and facilitates the bedside clinical care of Stroke/TIA, and other complex cerebrovascular patients to assure that all patients receive comprehensive and timely treatment interventions that are consistent with evidence based care as defined by Clinical Practice Guidelines. Works collaboratively with the multidisciplinary health team, is responsible for providing comprehensive care to patients by assessing, planning, implementing and evaluating care. Position focuses mainly on inpatient care, though may also work in outpatient clinical practice. This position requires some travel between acute system hospitals and provides general neurology and acute stroke care to Emergency Department patients, Observation Unit patients and inpatients, specifically cares for patients with neurological disease and/or stroke. May work at multiple sites on any given day depending on patient volume. It may be necessary to spend time at 4 sites during the day. This position will participate in virtual rounds with the neurology attending via a telemedicine mobile unit. Participates in patient and family teaching. Provides leadership by working cooperatively with health care team members in maintaining standards for professional practice in the clinical setting. Supports delivery of evidence-based acute stroke assessment and management. Provides expert nursing consultation and practice oversight. Develops and delivers acute stroke continuing education programs. Participates in performance improvement processes. Participates in Comprehensive Stroke research.
Responsibilities:
EDUCATION
Graduate of an accredited school for Nurse Practitioners
Masters of Science degree required
Maintains a current New York State Professional RN License and New York State Professional Nurse Practitioner License
Maintains Adult or Family Nurse Practitioner Certification
Meets credentialing requirements as appropriate through the Medical Staff office
Or graduate of an approved National Commission on Certification or Physician Assistants (NCCPA) and Committee on Allied Health Education and Accreditation of the AMA (CAHEA) accredited Physician Assistant training program
EXPERIENCE
Three (3) years of experience as a Nurse Practitioner preferred
Two (2) years of experience in the neurosciences preferred

CPR Certified
ACLS Certified
Management of intracranial hypertension
Cerebral perfusion studies
Ventriculostomy management
Acute cerebrovascular rescue
Ability to perform IV Thrombolysis
Competency in interpretation of CT and MRI of the central nervous system
Demonstrates excellent communication and customer service skills
Competency in leverage telemedicine equipment and virtual monitoring systems
