he Adult Protection/Child Family Foster Care Licensing Social Worker's primary purpose is to ensure the health and safety of individuals we serve and provide person centered planning with an effort to enhance individual lives and to promote happiness, independence, and autonomy. Social Workers must determine eligibility for the programs coordinated and monitored by the County, as well as assisting clients in making informed choices regarding services. This position will be offered at a starting wage of $55,700/annually and will be open until filled. Direct Service Duties and Responsibilities: Maintain caseload and provide ongoing case management including crisis management/Effectively interview and engage clients to assess and evaluate needs/Develop and complete assessments and documentation, including but not limited to, social histories, home assessments, and individual or family service plans to treat identified problems, and prepare intensive long or short-term treatment plans/Develop and implement safety plans for clients as needed. Process Duties and Responsibilities: Maintain accurate client records on computer system, prepares reports, and submits to proper personnel and/or agencies/Prepares and maintains progress notes, narrative summaries, reports, and other required case documentation in the file and in SSIS/Collaborates and consults with multidisciplinary and multi-agency teams in order to achieve desirable outcomes for clients/Attends continuing education programs and participates in in-service training. Adult Protection: Receive intakes and completes investigations on adults alleged to be in situations of abuse, neglect, self-neglect, or exploitation/Assesses each adult's social, emotional, physical, mental, environmental, and financial circumstances/Travels as needed to conduct home visits/Interview reported adults, caregivers, and alleged perpetrators with law enforcement and public health as needed/Determines each adult's mental capacity to consent to protective services. Child Family Foster Care Licensing: Provide orientation and resource linkage to new child foster care providers/Complete initial licensing and re-licensing requirements in a timely manner according to licensing rules, statutes, and applicable policies/Create and write home study reports for all licensed foster homes.
