Job SummaryUnder minimal supervision, plans, organizes and implements psychiatriccase management for clients and families. This work predominantly includesidentifying and connecting patients to other psychiatric services in thecommunity to assist with their ongoing behavioral healthcare. The work mayalso include a small amount of clinical work that would include assessing thepsychological needs of patients and families under a multi-cultural andtrauma-informed lens, consistent with the spirit of evidence-based practicessuch as cognitive behavioral therapy, acceptance and commitment therapy,and motivational interviewing.
Essential Functions:1. Functions as a social worker on the multidisciplinary inpatientconsultation-liaison psychiatry team, who sees clients on the medical,surgical, intensive care, pediatric, and labor & delivery services,and provides additional support to the Psychiatric Emergency Services(PES) when required. Social worker will provide coordination of care anddischarge planning to address the behavioral health needs of the client andfam2. Utilizes clinical assessment and diagnostic skills to engage in care plandevelopment with clients and families.3. Coordinates transfers and/or referrals to various levels of psychiatriccare including, but not limited to, internal adult and child inpatientpsychiatric units, community adult and child inpatient psychiatric units,acute treatment units (ATUs), crisis stabilization units (CSUs),partial hospitalization programs (PHPs), intensive outpatient programs(IOPs), and outpatient behavioral health clinics (including IntegratedBehavior4. Will call centers for availability, assure all necessary medicallabs/tests needed have been completed, complete and send the clinicalpackets necessary for transfer to community programs, communicate theoutcome with the primary medical team, and coordinate the transfer of theclient(s) from our hospital to the appropriate clinical location.5. Must be able to provide clients and their families with resources andreferrals to psychiatric services in the community (including, but notlimited to the above-mentioned programs)6. Will work in collaboration with our addiction consult service socialworkers to determine the most appropriate disposition for clients with dualdiagnoses (psychiatric and addiction diagnoses).7. Opportunities can be made available to strengthen clinical relationshipswith community psychiatric programs via site visits.8. Will be asked to participate in collection of data/metrics for qualityimprovement projects.9. If time allows and case management duties have been completed, there willbe some opportunity to help proactively screen new admissions to themedical/surgical/pediatric/intensive care services/PES for psychiatric needs.10. If time allows and case management duties have been completed, there maybe opportunity to provide psychotherapy to clients ex eriencing depression,anxiety, acute stress disorder, post-traumatic stress disorder,adjustment disorder, grief and bereavement. There will also be someopportunity to complete safety assessments for clients that screen positivefor high risk of suicide using the Columbia-Suicide Severity Rating Scale(C-SS11. Documents accurately and timely. Responds to patient and referral needs,maintains a safe environment of care and ensures the maintenance of patientconfidentiality.12. Promotes positive interpersonal (customer) relationships with fellowemployees, physicians, patients and visitors. Treats these individualswith courtesy, dignity, empathy and respect; consistently displayscourteous and respectful verbal and non-verbal communications.13. Adheres to, complies with and demonstrates support for the mission andvalues of Denver Health.14. Ensures confidentiality of patient information by creating and maintaininga secure and trusting environment by not sharing information learned on thejob, except when necessary in the performance of the job responsibilities orto improve a patient's care.15. EducationMaster's Degree from a certified Master's program in psychology,counseling, social work, or other related human services degree.Required
