About the job:
The Medical Social Worker (MSW) routinely assesses the needs ofparticipants and families and determines the most appropriate and effectivesocial services interventions to meet participant goals. Based on theassessment and/or evaluation of the participant, the Social Worker worksin coordination with Social Work Case Managers, Behavioral HealthSpecialist, Chaplain, contracted providers, and others to implement theplan of care and works directly with the Social Work Case Manager to providepsych/social interventions and takes the lead for acute and complex issuesrequiring a higher scope of practice. The Social Worker acts as an educationresource for the PACE team and supports the organizations goal to providehigh-quality, person-centered care to all participants.
Pay: $70k-$75k/Annually

Master's Degree in Social Work from an accredited school required.Must be licensed where applicable by the StateMust have a valid driver's license, proof of insurance and have means oftransportation.Must have medical clearance for communicable diseases and up-to-dateimmunizations before having direct participant contact.Must have a current and unrestricted Colorado Licensed Clinical Social Worker(LCSW)
Preferred Skills:
A minimum of two years' experience working with frail or elderly populationrequired. Experience working on a multi-disciplinary team in a hospital,nursing home or community-based setting is preferable.
Essentials:
Acts as and engaged member of the Interdisciplinary Team (IDT). Regularlyattends meetings and provides meaningful input.Performs in-person initial assessments for enrolling participants to obtain acomplete psychosocial history, which may include descriptions of cognitivestatus, social supports, family dynamics mental health and substancedependency and other issues and needs. Coordinates with the IDT, CareTeam, and Social Work Cas Manager to develop a comprehensive plan of carefor each participant.Reassesses participants on a semi-annual and as-needed basis and coordinateswith the IDT, Care Team, and Social Work Case Manager to update the planof care for each participant.Works in close coordination with the Social Work Case Managers and isresponsible for the psychosocial wellbeing of participants. Provides ongoingsupport and education to participants and family regarding a variety ofissues, including but not limited to: the aging process, dementia,grief and loss, end of life, disease processes, difficult familydynamics and changing roles, PACE model and PACE health services.Identifies, provides and/or coordinates for the medically-relatedsocial, psychological and spiritual needs of the participants. Implementssocial work interventions, case management, or psychosocial support. Asappropriate, may delegate these interventions to the Social Work Case Manager.Support the Social Work Case Manager and Care Team with acute and complex casemanagement.Acts as facilitator for meetings with pa ticipant, family, caregivers,and community agencies to clarify, or problem solves issues regarding theplan of care. Mediates discussions between all parties.
