We also provide practicum supervision, license supervision, and continuing education credits.Responsibilities for Senior Case Manager:Provides case management - Community Psychiatric Support Treatment (CPST) services and Therapeutic Behavioral Services (TBS) to persons with severe mental illness in an outpatient facility and in the community. May provide treatment to persons with substance use disorder as a secondary diagnosis.Initiates, coordinates, and monitors evaluations necessary to identify each client's needs in areas to include: living arrangements, financial issues, benefit status, legal status, physical/psychiatric health, activities of daily living, vocational/educational, social supports, leisure and recreation.By assertive outreach, attempts to engage the client in identifying and expressing his/her own needs, offers services to assist in meeting these needs, and re-assesses outcomes with the client on a regular basis.Gains knowledge of the following: mental health treatment and basic needs resources, informal and formal linkages with agencies that provide services, the client's use of services, and applicable costs of use, and the opportunities for alternative resources.Serves as a liaison with other service staff.Delegates services to secondary and supportive service providers; and monitors and evaluates the outcomes of these services.Assists the client to achieve goals and maximize independence and productivity through support, teaching, and training in the use of personal and community resources.Reviews and monitors services and activities and documents progress according to organization processAssists in onboarding and initial training of new care managers.Serves as a resource to less experienced care management staffMay serve as a representative of GCB in partnerships and shadowing experiences, such as CIT police shadowing.Provides services to individuals in Cincinnati/Hamilton County or Amelia/Clermont County.Position Requirements:Bachelor's or Master's degree in a Human Services field or High School diploma and a minimum of three years experience in Behavioral Health,Mental Health or a Human Services related field and two years of experience as a Mental Health Care Manager.Experience in social services and behavioral healthPosition requires frequent driving of the employee's personal vehicle.
