JOB REQUIREMENTS: Overview Provides compassionate, culturally sensitive,holistic nursing care to patients in a clinical setting, supporting theMercyhealth nursing mission, practice values and strategic plan. Aresource and a role model for staff by coordinating clinical activitiesto ensure quality patient care is delivered and maintained. Promotesexcellence in customer services with internal and external customers;maintains effective operations, patient/customer satisfaction, andpositive patient outcomes. Develops, plans, coordinates, and implementsorientation, continuing education, and training programs and skillscompetency validation that result in qualified and knowledge staff.Other duties as assigned. Responsibilities Demonstrates broad knowledgeof rules and regulations governing hospice. Actively participates in thequality improvement process to evaluate program progress and assistsstaff in identifying areas of needed improvement. Assists in thepreparation for accreditation and certification visits related tohospice. Coordinates communication between staff and other providers.Receives patient and physician phone calls and relays information toappropriate staff members including documentation in the medical record.Obtains and records physician orders and communicates orders toappropriate staff members for implementation. Coordinates referrals withthe Intake Department. Reviews and collaborates on incoming referralsfor appropriateness of services and coordinates timely initiation ofservices. Coordinates insurance authorizations with insurance casemanagers and obtains orders for services accordingly and informs staffmembers and billing department. Understands all pay sources andcompletes all necessary documentation and communications required by thepay source. Responsible for the clinical guidance and development ofstaff utilizing a variety of supervisory techniques including caseconferences and joint home visits. Identifies staff development needsand develops actions plans to meet those needs to ensure competency ofstaff. Provides staff with education commensurate with advances skilllevel and mentors personnel on expert clinical care of the patient.Assists with providing resources/education for new clinicalcompetencies. Performs regular skills competency validation with allstaff. Shares responsibility for program outcomes as defined bystrategic plan within the agency and in conjunction with the healthsystem, the QI program and the agency budget Participates in hiring,evaluations, disciplinary, and termination process for staff. Identifiesstaff performance problems, provides counseling and develops workperformance improvement plans. Assures fair and equitable distributionof assignment work load to the staff.
