Job Description/Essential Functions:

Horizon House is Pennsylvania's oldest psychiatric rehabilitation agency. Since 1952, we have been dedicated to helping people with mental illness, drug and alcohol addictions, developmental/intellectual disabilities, and homelessness to stabilize their lives and to help them live as independently as possible in the community. With over 100 locations throughout South and North Eastern Pennsylvania and the state of Delaware, Horizon House provides services to a diverse population focusing on individual strength and choice.We are seeking a part-time Rehabilitation Counselor to provide support, education, counseling and guidance to child(ren) or adolescent(s) who has been identified as having mental health or developmental disabilities. This position is responsible of the following functions:
To provide support, education, counseling and guidance to child(ren) or adolescent(s) who has been identified as having mental health or developmental disabilities.To assist with client movement, goal achievement and local community interaction.To implement the overall mission, values and beliefs of Horizon House in all aspects of the job.To provide one-on-one behavioral intervention in the home, school, or community sittings in accordance with strategies identified in the Treatment Plan.To participate in developing goals with the client, for inclusion in the individualized service plan.To maintain and submit required program reports and documentation, including progress notes describing the client contact and how it relates to treatment goals, service plan reviews, incident reports and medical/appointment logs.To serve as an advocate on behalf of the clients and encourage them to assume a proactive role in their rehabilitation/transition process.Job Qualifications:
Bachelor's Degree in Human Services/Social Services, Healthcare or related field. At least 1 year of experience working directly with children; or H.S. Diploma or GED equivalent plus 12 behavioral health college credit hours from an approved educational program plus a minimum of 2 years experience (paid or unpaid) in a human service field. The experience must involve direct contact with the individual receiving services (i.e. coaching, teaching, case management, etc.)Experience in residential or other services to children with MR/MH preferred.Good communication and counseling skills.Ability to work independently and as part of a focused team.Proficiency in various computer technologies including Electronic Health Record, Outlook, MS Word and Excel.Fee-for-service experience required and ability to maintain daily productivity standards using an Electronic Health Record.Possess a valid driver's license with an acceptable driving record.Scope:
