The Physical Therapist is responsible for the evaluation,planning and implementation of a treatment plan addressing thefunctional needs of a variety of patient diagnoses. ResponsibilitiesIntegrates subjective and objective data to establish a treatment plan.Utilizes exercise, physical agents, and manual techniques to addressdeficits. Maintains a thorough record of services provided withappropriate coding and billing. Communicates with other disciplines andreferring providers to enhance outcomes. Performs thorough and timelyelectronic documentation Strengths in care delivery as evidenced by peerreview feedback; consistently demonstrates advanced evaluation andtreatment techniques Participates actively in the PI and CQI processAssists in the orientation, training and mentoring of new partnersParticipates in cost saving initiatives within the department Takes anactive role in the performance improvement process Maintain a highstandard of patient care and provide effective communication betweenphysical therapists, assistants and the physician group Plans andconducts lectures and training programs on physical therapy and relatedtopics for medical staff, students and community groups. Education andExperience Graduate of an accredited school of Physical TherapyCertification/Licensure Licensed as a Physical Therapist in WI, IL orboth if required Approved CPR or BLS certification required within 90days Special Physical Demands The Special Physical Demands areconsidered Essential Job Functions of the position with or withoutreasonable accommodations.
