Summary
The Project Coordinator of Older Adult Protective Services Pathways to Safety Program will record and analyze data to determine the effectiveness of the project, complete performance measurement reports as required by Program Management, and handle a caseload of elder victims. In addition, the Project Coordinator will make regular follow up communications in the form of both face to face visits and phone calls with the elder victim and their family or support system, ensure the stability of services put in place following the investigation, make referrals for new services as needs change, provide information on resources available in the community, provide trauma informed education, and provide informal support to the elder victim and any caregivers during this time of transition following victimization.
Essential Duties and Responsibilities:
Complete monthly performance measurements to Supervisor. Provides case work services to protective service client population. Conducts the intake function on potential cases, identifies individuals appropriate for this program, and makes referrals for those individuals to appropriate services. Conducts individual assessment at client's home to assess risk and client's needs. Adjusts care plan for crisis intervention services, safety planning and if necessary, arranges for on-going services depending on circumstances of case. Works closely with community agencies and organizations including first responders, community safety personnel and medical professionals to identify needs, locate resources, and set up service network to meet needs. Identifies community resources available to meet service needs of elder abuse victims. Maintains case records on all clients; collects information for program monitoring and evaluation and completes necessary statistical documentation. Assists with other program activities as assigned.

Ability to manage program participants and intervene as appropriate according to the Agency vision and values. Ability to verbally and non-verbally communicate in a clear and concise way. Capable of working individually, as well as in groups. Ability to employ effective time management skills. Ability to determine strengths and weaknesses in a client system to service a strengths-base perspective. Ability to exhibit empathetic and intellectual listening to employ active and meaningful communication and needs assessment in a client system. Proficient use of laptop/computer with Microsoft Office Software, program databases, internet, and smart phones.
Education/Experience Requirements
Bachelor's degree in social work, psychology, sociology, gerontology or other behavioral science, plus a minimum of 1 year of human services experience and/or case management experience working with older adults.

Any equivalent combination of education and experience that meets the required knowledge, skills and abilities.
Certificates, Licenses, Registrations
Act 33, 34 and FBI clearances.

Screening for debarment prior to hire and monthly during employment through both the Office of Inspector General and System for Award Management databases.

Valid driver's license and access to a reliable vehicle, with proof of insurance coverage.
