JOB REQUIREMENTS: Duties Assigns codes to documented patient careencounters (inpatient facility and/or professional services) coveringthe full range of health care services provided by the VAMC. Appliesadvanced knowledge of medical terminology, anatomy & physiology, diseaseprocesses, treatment modalities, diagnostic tests, medications,procedures as well as the principles and practices of health servicesand the organizational structure to ensure proper code selection.Selects and assigns codes from the current version of several codingsystems to include current versions of the International Classificationof Diseases (ICD), Current Procedural Terminology (CPT), and/orHealthcare Common Procedure Coding System (HCPCS). Adheres to acceptedcoding practices, guidelines and conventions when choosing the mostappropriate diagnosis, operation, procedure, ancillary, or E/M code toensure ethical, accurate, and complete coding. Applies codes based onguidelines specific to certain diagnoses, procedures, and other criteriaused to classify patients under the Veterans Equitable ResourceAllocation (VERA) program that categorizes all VA patients into specificclasses representing their clinical conditions and resource needs.Monitors ever-changing regulatory and policy requirements affectingcoded information for the full spectrum of services provided by theVAMC. Timely compliance with coding changes is crucial to the accuracyof the facility database as well as all cost recovery programs. Performsa comprehensive review of the patient health record to abstract medical,surgical, ancillary, demographic, social, and administrative data toensure complete data capture. Patient health records may be paper orelectronic. Assists facility staff with documentation requirements tocompletely and accurately reflect the patient care provided; providestechnical support in the areas of regulations and policy, codingrequirements, resident supervision, reimbursement, workload, acceptednomenclature, and proper sequencing. Insures provider documentation iscomplete and supports the diagnoses and procedures coded. Consultsdirectly with the professional staff for clarification of conflicting orambiguous clinical data. Reports incorrect documentation or codes in theelectronic patient health record.
