Job Description:
We are seeking a detail-oriented and organized Medical Records Specialist to join our
healthcare team. The Medical Records Specialist plays a crucial role in maintaining
accurate and confidential patient records in compliance with regulatory requirements.
This position requires strong attention to detail, proficiency in medical terminology, and
the ability to work effectively in a fast-paced healthcare environment.
Key Responsibilities:
Record Management: Organize, maintain, and update patient medical records,
including electronic health records (EHR) and paper charts, ensuring accuracy,
completeness, and confidentiality.
Data Entry: Enter patient information, diagnoses, treatments, and other medical
data into electronic health record systems or other database systems accurately
and efficiently.
Release of Information: Process requests for release of medical records from
patients, healthcare providers, insurance companies, and other authorized
parties, ensuring compliance with privacy regulations (e.g., HIPAA).
Record Retrieval: Retrieve patient records for healthcare providers, clinicians, and
other authorized personnel as needed for patient care, treatment planning, and
billing purposes.
Quality Assurance: Review medical records for completeness, accuracy, and
compliance with regulatory standards. Flag any discrepancies or errors for
correction.
Coding and Indexing: Assign appropriate medical codes to diagnoses,
procedures, and treatments using standard coding systems (e.g., ICD-10, CPT),
ensuring accuracy and consistency.
Auditing: Conduct periodic audits of medical records to ensure compliance with
documentation standards, coding guidelines, and regulatory requirements.
