The Health and Social Services Manager will lead a care management team while overseeing day-to-day clinical operations and health plan clinical initiatives to support achieving quality and affordability targets. The HSS Managers will support a team within different regions in Northern Indiana. Responsibilities will include process improvement, execution of clinical decision processes, and team management, to include performance management and implementation of program initiatives.

Primary Responsibilities:
Selects, manages, develops, mentors and supports staff in designated department or region Develops clear goals and objectives for performance management and effectively communicates expectations, and holds the team accountable for results In order to meet the unique needs of our members, have an intimate understanding of the contractual requirements Identify, select, structure, and prioritize process improvement projects, ultimately implementing changes to meet program requirements Conducts monthly audits of staff and participates in all other required audits and business monitoring Ensures standardized execution of workflow processes, including conducting performance audits, quality reviews, and compliance adherence Participates in training and coaching of direct reports as needed Collaborates across Optum and UHG and interacted with Medical Directors, Senior Leaders, Quality, and other stakeholders

Required Qualifications:
Active and unrestricted RN license in the state of Indiana or Master's level social worker 5+ years of experience in providing care coordination to older adults Experience working within the community health setting in a health care role Experience facilitating and working in coordination with an Interdisciplinary Team Experience coaching or mentoring staff Intermediate level of experience with Microsoft Word and Excel Proven ability to navigate a TEAMs environment Resident of Indiana
Preferred Qualifications:
1+ years management experience Case Management experience Experience with Long Term Service and Supports or Waivers Program Background in managed care
