Horizon House is Pennsylvania's oldest psychiatric rehabilitation agency. Since 1952, we have been dedicated to helping people with mental illness, drug and alcohol addictions, developmental/intellectual disabilities, and homelessness to stabilize their lives and to help them live as independently as possible in the community. With over 100 locations throughout South and Northeastern Pennsylvania and the state of Delaware, Horizon House provides services to a diverse population focusing on individual strength and choice. We are seeking qualified candidates for the position of Licensed Practical Nurse (LPN).
Purpose:Coordinate the healthcare needs of the person with behavioral healthcare concerns. Deliver care toparticipants utilizing the nursing process including the planning of care, implementation, and resident and familyeducation. Instruct, counsel, and support assigned participants, focusing on participants' independence andindividual needs. Maintain an environment, which encourages proactive decision-making and growth for eachparticipant.
Supervision Received:* Receives medical direction from consumers' prescriber.* Receives general direction from the Program Manager/Team Leader and Director of Residential Services.

Qualifications:
Completion of a certified nursing course as evidenced by a license as a Licensed Practical Nurse(L.P.N.). Experience in community nursing and mental health with co-occurring disorders.Prior experience in serving psychiatric needs in mental healthComprehensive knowledge of psychiatric clinical and psychosocial rehabilitation practicesStrong interpersonal skills, organization, and analytical skills.Must be available for emergency on-call service.Excellent written and verbal communication skillsValid driver's license requiredAbility to work independently and as part of a focused teamScope:
