Job Description:
+ + Follow the plan of care for patient/family care and services.
Outlines admission plan of care with the IDT and RN Case Manager.
Provides nursing care as determined by the plan of care and the RN Case Manager.
Educates patient/family and other primary caregivers, in appropriate care techniques.
Serves as the patient/family advocate by maintaining regular communication with ALF staff, long-term care facility staff, and RN Manager.
May participate in providing bereavement support to families and associated bereavement activities as appropriate.
Follows RN care plan appropriately/reports to RN case manager on each patient.
Monitors the recertification/reauthorization process and provides input for this process.
Qualifications:
Currently licensed to practice nursing in the state
A minimum of two years of nursing experience in acute-care hospital nursing in either medical, surgical, or oncology unit or other commensurate experience.
