Summary:
The Licensed Practical Nurse is responsible for the provision of nursing care to a designated group of patients under the direction of a registered professional nurse. The Licensed Practical Nurse adheres to the course of treatment as established by the RN. The Licensed Practical Nurse confers and collaborates with all members of the patient care team in the execution of the care plan for a specified patient, reports all findings to the RN case manager or clinical leader and reports significant abnormal findings to the physician. The Licensed Practical Nurse may perform duties in other areas of the Agency that are compatible with licensure requirements and as directed under the supervision of an RN. Performance reflects the mission and values of Home Care & Hospice.
Responsibilities:
Under the supervision of an RN performs clinical care as directed and within the scope of discipline specific clinical practice standards.
Recognizes symptoms and changes in the patient’s condition and provides feedback to the primary clinician, clinical leader, an/or physician to assist in the evaluation of care.
Performs complete, accurate and timely documentation within the electronic health record.
Maintains a safe environment complying with NLH policies and procedures; reports and directly addresses environmental hazards and violations of patient safety policy and/or protocol when involved or observed.
Participates in patient, caregiver, staff education.
Attends appropriate team meetings and communicates/collaborates with other team members, individuals, family members or caregivers.

BLS for Health Care Providers required
Valid driver’s license with reliable transportation and agency required auto liability insurance. Must be able to drive within assigned region as directed.
