You will become a part of highly regarded and successful community service lines that are an important part of WesleyLife's mission and ministry. You will be asked to continue our reputation of excellence in service, hospitality, and quality as you join a committed group of team members who are passionate about the mission of our organization. As our Lead Wellness Coach, you will provide personalized wellness coaching that helps to ensure members are able to continue the opportunity to pursue their joy while living in their own homes, receiving support and coaching to promote health and well-being. The purpose is holistic well-being, independence, safety and happiness!
The ideal candidate for the Lead Wellness Coach will have the following capabilities and characteristics.
An undergraduate degree in health and wellness, therapy, health administration, or a similar field of study with prior and diverse experience in a health and well-being-related position.
A passion for serving older adults, for individual advocacy and for holistic health and well-being.
Strong case management skills within the aging / older adult services is highly preferred.
Demonstrated knowledge of health and wellness, and medical services in the defined service area.
CPR/AED certification, or ability to become certified within three months of hire is required.
Must demonstrate and model health and well-being to create a high level of trust for members in their ability to manage their personal health and, in turn, advocate for them.
Dependability, reliability and consistent attendance at work are essential functions.
In summary, this This role oversees the creation and management of holistic health programs for the Continuing Care at Home initiative, ensuring members receive personalized wellness coaching. Responsibilities include designing programs, supporting wellness coaches, managing deployment of services, leading the coaching team, and maintaining relationships with members. The role involves advocating for members' needs, collaborating with care teams, documenting services, ensuring confidentiality, promoting healthy living, and coordinating with other services within WesleyLife. Additionally, the role involves seeking innovative ways to improve services and handling various projects and additional responsibilities as needed.
