Position Details:
Location: Latrobe, PA Supporting Locations: Excela Health Latrobe Hospital (1 Mellon Way, Latrobe, PA 15650)
Excela Health Westmoreland Hospital (532 W Pittsburgh St, Greensburg, PA 15601) Hours: 40 hours/weekly, full time. Monday through Friday, hours ranging between 8:00AM - 5:00PM
The Inpatient Care Coordinator (ICC) plays an integral role in the "high risk" member's journey towards better well-being. By serving as the link between members and appropriate health care personnel, the ICC is responsible for identifying the appropriate post-acute care setting ensuring efficient, smooth, and prompt health care services will be delivered to the member following the acute care episode. The position also provides emotional support to both the member and their relatives and provides medical advice during the discharge planning process.
Primary Responsibilities:
Own the member relationship and is the single point of contact for hospitals with on-site engagement. On-site engagement defined as: EMR access Access to patients Engagement with hospital CM, D/C Planers, Social Workers, & Hospitalists Access to hospital census is preferred Own prospective skilled nursing facility requests that originate from an in-person request while on-site or from a call direct to the ICC personal line Approve prospective requests within established parameters Forward requests to utilization management partners if outside authorization parameters or if unable to complete the request Perform other duties and responsibilities as required, assigned, or requested

Required Qualifications:
Active, unrestricted registered clinical license - Registered Nurse, Physical Therapist, Speech Language Pathologist, or Occupational Therapist 3+ years of clinical experience Proficient with Microsoft Office applications including Word, Excel and PowerPoint Willing or ability to support specific location(s) for on-site facility needs within 20-mile maximum radius of home location based on manager discretion. Reside within or near Latrobe, PA
Preferred Qualifications:
Experience working with an electronic health record (Cerner) 2+ years of Case Management experience Patient education background, rehabilitation, and/or home health nursing experience Experience working with geriatric population Knowledge of medical terminology Proven exceptional verbal and written interpersonal and communication skills Proven solid problem solving, conflict resolution, and negotiating skills Proven independent problem identification/resolution and decision-making skills
