The Home National Team (HNT) provides support to the patients-and theteammates who serve them-who perform dialysis at home instead of in a center.Home dialysis provides important clinical benefits for patients, is a keydriver of DaVita financial and operational performance and is a strategicpriority for the DaVita Village as support more patients succeeding on Homedialysis.
HNT's Home Innovation Lane is seeking a driven, organized, andcollaborative teammate to support the implementation, project management,and analytics for innovation initiatives, including supporting newenhancements to CWOW DaVita's Electronic Health Record platform.
This position reports to the Sr. Manager, Operations Innovations for DaVitaat Home.
Electronic Health Record (EHR) Enhancement Support
Collaborate with Home stakeholders to prioritize new functionalities withinDaVita's propriety EHR, "Center Without Walls" (CWOW)Develop materials to support the launch of Home CWOW initiatives to clinicsProvide outreach to clinic teammates around CWOW innovation pilotsManage document approvals through legal and compliance process and maintain anupdated database of approved materials for CWOW innovation projectsAid business owners in evaluating the ROI of their CWOW innovation projectsCollaborate with Home and IT stakeholders in the development of KPIs forinnovation and CWOW initiativesMeasure impact of CWOW initiatives after implementationMaintain repository of Home National Team and field teammate requests for CWOWenhancementsOnboard Home stakeholders around the Home CWOW process and expectationsDocument grooming and development meetings between CWOW and Home BusinessOwnersDesign slides to compellingly communicate the story and insights from CWOWinitiatives to internal audiences
Other Duties:
General support across other digital platforms including Physician-facingplatforms ("OneView"), Patient-facing platforms ("DaVita CareConnect"), and admissions platforms ("DaVita Placement Portal"), including:
Collaborate with Home stakeholders to prioritize new functionalities withinDaVita's other digital platforms for patients and physiciansAid in building and maintaining playbooks for Home team business owners acrossdigital toolsDevelop materials to support the launch of Home digital initiativesSupport operational functions for Home Patient Virtual Support Groups(VSGs), delivered on DaVita's patient app, DaVita Care ConnectManage document approvals through legal and compliance process and maintain anupdated database of approved materials for digital innovation projectsBuild and support the maintenance of a Home Digital Initiative metricperformance repositoryFacilitate communication between business owners and IT teams around Home ITand digital projectsAid business owners in the research and analysis to assess ROI on digitalinnovation projectsCollaborate with Home and IT stakeholders in the development of KPIs forinnovation and digital initiativesSupport awareness efforts to field stakeholders around DaVita's patientreferral platform , the DaVita's DaVita Placement Portal (DPP)
