  Job Description

  The Department of Mental Health (DMH), is seeking qualified professionals for the position of Human Service Coordinator Transitional Age Youth (TAY) to promote services that are clinically appropriate, least restrictive, and delivered close to home. The services are designed to be strength-based, recovery-orientated, and culturally/linguistically competent. The selected candidate advocates for client's rights with all community providers, agencies, families, court systems, employers, and significant others to ensure that client's rights are recognized and respected.

  The Human Service Coordinator I (TAY) provides assessment, outreach, support, service coordination and person-centered advocacy primarily to DMH eligible transitional age individuals by promoting DMH's philosophy of recovery, person-centered choices, and cultural competence by utilizing strength-based goal setting and through independent decision-making. Responsible for development, implementation and monitoring of individual service plans; assists individuals in securing services and performs all duties to reflect the highest level of respect and dignity for the persons-served.

  Please note: This position is full time 37.5 hours per week, Monday - Friday 9:00am-5:00pm with Saturday and Sunday off.

  Duties and Responsibilities (these are a general summary and not all inclusive):

  Develops comprehensive assessment of transitional age individual needs by interviewing the individual, family and/or LAR, significant others, obtaining records, gathering relevant written and verbal information from past and current service providers in order to determine the service needs.Develops, implements, and monitors the individual service plan (ISP) per DMH Regulations with the person-served and service providers, chairs and convenes meetings to review and modify the ISP and risk management plans when indicated; recommends and implements referrals and transfer of individuals to other programs and services as needed. The written ISP is culturally competent and reflects recovery-based, person-centered language and practices.Provides regularly scheduled outreach visits and recovery-based education to the person-served and family members (as appropriate); develops positive relationships with them; and provides education about DMH and non-DMH services to enable them to identify, accept, and obtain the necessary services including financial entitlements and health insurance.Utilizes and integrates the Management Health Information System (MHIS) data by entering and maintaining accurate data such as demographic, service tracking and billing information, documentation of encounters and progress notes in accordance with DMH and MHIS business rules and Regulations.Maintains records for each assigned individual by requesting all relevant information; thoroughly assesses, researches and documents each individual's history including risk; documents termination summaries, risk management plans, meeting minutes, and by regularly updating changes in records to ensure completeness and accuracy.Facilitates and/or participates in meetings such as hospital treatment team and discharge planning, program reviews, and Clinical Reviews in order to monitor service plans and make recommendations for other services that will meet individual choice and needs.Assess potential crisis and risk level of assigned individuals and collaborate with the individual, crisis team, and service providers to facilitate appropriate planning. Assists individuals in acute crisis by facilitating timely connection with appropriate crisis intervention service. Refers individuals to programs and services and facilitates the process by completing the required forms, advocating for the individual with programs and services, educating the individual about procedures, and by transporting when necessary to the required services to ensure the implementation of service plans. Assists individuals to develop and maintain a supportive network in the community, including family, significant others and friends that results in meaningful social relationships. Collaborates and advocates with school systems, community, and state agencies, as needed.Effectively prepares for and uses regular supervision to process and resolve issues and concerns related to persons served and services in a positive and productive manner; attends training and inservice educational opportunities to enhance skills and improve quality of care to persons served.Serves as a representative of the Department of Mental Health in order to foster public relations by maintaining good working relationships with service providers, community agencies, hospitals, and individuals, to promote cooperative planning and quality care for persons served.Performs job duties in a professional manner by exhibiting a positive attitude and adapts to changes in philosophy, policy, and procedure in a flexible manner. Complies with HIPAA regulations and confidentiality standards regarding the disclosure of PHI information.Performs other duties as assigned.

  Required Qualifications:

  Knowledge of clients' needs, and characteristics associated with DMH Priority clientsSkills and knowledge of counseling strategies and techniquesSkills in making productive referrals, negotiating assertively from an advocacy-based client centered perspective with a variety of public service agency professionalsConsiderable ability to work independently on assignmentsAbility to work non-judgmentally with clients whose belief system may be predicated on values incongruent with those of the Case ManagerArticulates clearly and concisely both in verbal and written modesKnowledge of principles and practices of crisis intervention and risk managementAbility to exercise discretion in handling confidential information

  Preferred Qualifications:

  Strong and effective interpersonal skills, including the ability to listen well, relate to others and demonstrate sensitivity and respect for individualsDemonstrated commitment to serving a diverse population with cultural competence and sensitivity, as well as the ability to work with a wide range of constituencies with diplomacy and tactProficiency in Microsoft Office applications including Outlook, Word, and Excelvalid drivers licenseGiven the population served, bilingual or multilingual fluency

  Qualifications

  Applicants must have (A) at least three (3) years of full-time or equivalent part-time, professional experience in human services work or social work or (B) any equivalent combination of the required experience and substitutions below.

  Substitutions:

  I. A Bachelor's degree or higher in a related field may substitute for two (2) years of the required (A) experience.

  Based on assignment, travel may be required. Incumbents who elect to use a motor vehicle for travel must have a current and valid motor vehicle driver's license at a class level specific to assignment.

  Based on assignment as Qualified Intellectual Disabled Professionals, within the Department of Developmental Services, a Bachelor's degree or higher in social work, psychology, sociology, counseling, counseling education, education of the physically or emotionally handicapped, education of the multiple handicapped, education of the learning disabled, human services, rehabilitation, rehabilitation counseling, nursing, recreation therapy, art therapy, dance therapy, music therapy, physical education or other a related field is required.
