Job Description
The Department of Mental Health (DMH), is seeking qualified professionals for the position of Human Service Coordinator to promote services that are clinically appropriate, least restrictive, and delivered close to home. The services are designed to be strength-based, recovery-orientated, and culturally/linguistically competent. The selected candidate advocates for client's rights with all community providers, agencies, families, court systems, employers, and significant others to ensure that client's rights are recognized and respected.
The Human Service Coordinator I provide case management services to a caseload of adult DMH clients. The Human Coordinator I will promote each client's recovery through the planning and implementing of a well-coordinated array of services, treatments, and benefits based on the client's preferences, strengths, and needs and on the availability of resources.
Please note: This position is full time 37.5 hours per week, Monday - Friday 9:00am-5:00pm with Saturday and Sunday off.
Duties and Responsibilities (these are a general summary and not all inclusive):
Carries a case management caseload and will help clients locate and maintain housing, financial assistance, medical, mental health and dental services, vocational and educational resources, socialization opportunities and transportation, to support clients successfully transition to and placement in the community.Completes a comprehensive assessment of client needs in accordance with DMH regulations 104 CMR 29.00.Completes risk assessments as required.Develop Individualized Service Plan for clients on caseload to insure coordination and monitoring of all aspects of treatment for each client. ISP to be completed within timelines established in DMH regulations.Endorses a person centered approach in all planning activities.If necessary, provides outreach and crisis intervention to clients to assess mental health needs and insure proper psychiatric treatment.Maintains and initiates extensive liaison work with community agencies including mental health services, court, welfare, and social security, to maximize the quality of life for clients.Collects standardized client related data for research, evaluation, or administrative purpose.Maintains clients' records with up-to-date client information, progress notes and ISP, in order to insure coordination and adequate treatment services.Ensures timely entries in electronic medical records.Visits clients in their homes or usual place of residence and monitors the safety of the environmentFacilitates referral of clients to programs best suited to their service needs. Coordinates transitions to various community programs.Safeguards human rights of clients through familiarization with providers and domestic environment, support and advocacy and reporting of human rights.Performs related duties as required.

Qualifications
First consideration will be given to those applicants that apply within the first 14 days.
Applicants must have (A) at least three (3) years of full-time or equivalent part-time, professional experience in human services work or social work or (B) any equivalent combination of the required experience and substitutions below.
Substitutions:
I. A Bachelor's degree or higher in a related field may substitute for two (2) years of the required (A) experience.
Based on assignment, travel may be required. Incumbents who elect to use a motor vehicle for travel must have a current and valid motor vehicle driver's license at a class level specific to assignment.
Based on assignment as Qualified Intellectual Disabled Professionals, within the Department of Developmental Services, a Bachelor's degree or higher in social work, psychology, sociology, counseling, counseling education, education of the physically or emotionally handicapped, education of the multiple handicapped, education of the learning disabled, human services, rehabilitation, rehabilitation counseling, nursing, recreation therapy, art therapy, dance therapy, music therapy, physical education or other a related field is required.
