This is a highly responsible professional position requiring expertise in the Prevention & Community Services Program Office Hope Florida: A Pathway to Prosperity program. This position requires extensive knowledge of program concepts, Community engagement and quality reviews. The individual occupying this position is responsible for ensuring that policies and procedures of the program as assigned, are administered correctly. This position will assist Hope Navigators in working with customers to identify and overcome barriers to self-sufficiency. This is accomplished by maintaining close relationships with community stakeholders, strategic partners, and internal Department staff. Some travel will be required to Pensacola, Fort Walton Beach, Tallahassee, and Panama City Beach.

Essential Job Functions
Compiles statistical reports and prepares correspondence as requested.
Formulates an appropriate plan of action that addresses identified areas of need based on outcomes of the assessment.
Interprets, coordinates, and communicates program policies and procedures developed by the State Program Office to Operations staff on a continuing basis.
Develops, interprets, coordinates, and communicates program policies and procedures specific to Hope Florida: A Pathway to Prosperity staff on a continuing basis.
Identifying and maintaining a working knowledge of available community resources, in conjunction with the Partner Network team, to meet the individual needs of each family and ensures the connection of families to relevant community resources and support.
Provides professional assistance and consultation to Hope Florida staff to resolve problems arising in program eligibility, service provision and policy implementation.
Provides technical assistance, data support system and reports to the Hope Florida team.
Develops program information, data reports and analysis of process and performance information on a regular basis for distribution to staff and the community regarding program objectives, policies, changes, etc.
Serves as a program liaison to other DCF programs by providing program information and keeping them informed of various issues and decisions.
Assists in preparation of statistical and data analysis reports to the department and federal agencies; maintain familiarity and proficiency with statistical concepts as they relate to Hope Florida by case reviews, providing technical and assessment evaluations.
Provides technical assistance on the phone and on site to Hope Navigators.
Assesses, plans, develops, organizes, schedules, and delivers pre-service, in-service, and community training for the Hope Florida team members, and other training modules based on operational needs and requests within the Region.
Develops specific policy and procedures for special projects and programs. Assures identification and development of community resources for utilization by Hope Navigators.
Coordinates policy/procedural matters as appropriate.
Develops Hope Florida program goals, objectives, and assists in developing operational plans including Corrective Action Plans. Documents all pertinent information and ensures that all appropriate forms are completed and uploaded into the Dynamic program data tracking system accurately and timely.
Provides follow-up and monitors connections to service delivery via Dynamic system.
Preparing correspondence, case notes, narratives, technical reports, notifications, and related documents using computer-based applications.
Completes case reviews and completes data analysis to provide technical assistance where required to assigned units.
Responds to ad-hoc reporting requests and works with other programs and state agencies on data requests and reports as assigned and prioritized by unit management.
Conducts trainings for internal and external partners as requested.
Actively participates in ongoing cross-program training and team meetings.
Attends Multi-Disciplinary Team meetings and provides assistance as needed.
Building and maintaining relationships with respect, trust, sensitivity and confidentiality to client, coworkers and community stakeholders and partners.
Provides assistance on special projects as assigned.
Performs other related duties.

Required Minimum Qualifications:
Bachelor’s degree from an accredited college or university or equivalent, four (4) years of professional experience in data analysis, quality improvement, policy development, community engagement, social services, or community outreach.

Valid Florida Driver’s License.
At least one year experience as a Hope Navigator or in a position conducting quality reviews.
Able and willing to travel within the region and/or to stay overnight outside the region when required.
Preference will be given to candidates who meet the following:
Bachelor’s degree from an accredited college or university in a Human Services field, data analytics, or a related field.

Note: Case management experience is defined as experience with conducting assessments, preparing plans, making referrals, implementing plans and follow up with monitoring and evaluation of those plans
Software Proficiency:
Knowledge of the principles and techniques of effective communication, office procedures and practices
Basic arithmetic.
Ability to deal with the public in a tactful, courteous and effective manner; speak clearly and correctly; listen effectively; record and evaluate data relating to applicant employment or assistance for health care or community services; understand and apply applicable rules, regulations, policies and procedures relating to applicant employment or assistance for health care or community service.

Knowledge of Microsoft Office and ability to learn other software as needed.
