Shift: Monday - Friday, 8:30a-5p availability required, occasional weekends/holidays required based on department need.Summary:Under the supervision of the registered nurse provides personal care services; including bathing, dressing, and feeding.Assists with patient mobility, using assistive devices if necessary.Performs household duties as needed to improve / maintain the patient’s health status; including bed making, laundry, shopping, and meal preparation. Documents care provided.Position Responsibilities:• Knowledgeable of and supports the plan of care under the direction of the RN.• Performs personal care and household chores to/for the patient as established in the plan of care in order to maintain patient’s health status.• Submits all documentation (care plan, time sheets, API, etc) accurately and timely.• Identifies and communicates changes in the patient’s status to RN and responds appropriately.• Reinforces education of the patient and family / responsible party regarding individual care needs, wellness, and safety issues.• Practice standard precautions and patient safety principles.• Identifies and participates in performance improvement activities. Maintains departmental level competencies.Position Qualifications Required / Experience Required: Home Health Aide or Nurse Aide experience preferred. Required Education:
High School Diploma or equivalency preferred.
Successful completion of State Certified Home Health Aide course.
Training/Certifications/Licensure:
Home Health Aide Certification-NJ Board.
Valid, unrestricted driver’s license.
