This Health Engagement Nurse role is a field based position with Washington University in St. Louis, MO and is an exciting opportunity for an experienced nurse to work with the University's employee and dependent population to promote engagement in clinical, behavioral health and disease management programs and help improve the member's healthcare experience through better healthcare decisions.
The ideal candidate will have experience providing health education and condition management guidance on an individual and group level. The nurse liaison will help empower members to make better healthcare decisions and connect them with the appropriate resources. Candidate will utilize analytic data to improve employee engagement, affordability and decision making to help deliver high impact results. This position focuses on health education, behavior modification, health care consumerism with UnitedHealthcare and Optum Health services, as well as, support engagement in the Washington University wellness program and the WUCare Network of providers.
Primary Responsibilities:
Educate members on health education topics and condition management for blood pressure, diabetes, obesity, and other conditions in a formal and informal setting Share medical plan programs and resources with members to help them better manage their health Refer members to available clinical programs Referral of members to WUCare (Washington University network of providers) for their medical care needs Create, manage and implement health, well-being and condition management programs based on population needs Monitor and analyze customer specific health analytic data to develop targeted data driven strategic plans and programs May perform clinical assessments, non-invasive biometric screenings and associated health education and counseling with members Clinical interface with facilities and providers for resolution of issues concerning members, benefit interpretation, program definition and clarification Extensive knowledge of and application of the social determinants of health to make community resource referrals
Candidates must be located within the St. Louis Metropolitan area. This position is 100% onsite with the client and requires travel locally to multiple University locations. This position is a 40 hr work week Monday - Friday; however, schedule flexibility is required (arrival/departure times).

Required Qualifications:
Registered Nurse with active and unrestricted license for state of residence 5+ years clinical experience in a hospital, acute care, direct care or care management Proficient in Microsoft Word, PowerPoint and Excel including the ability to navigate a Windows environment Comfortable with individual and group presentations for small and large audiences and have solid presentation skills with proven experience delivering group presentations and individual coaching
Preferred Qualifications:
Bachelor's degree in Nursing 5+ years' experience in managed care with significant program management experience or case management experience and / or Case Management Certification (CCM) Experience providing mental / behavioral health education and providing information, resources and referrals to groups and individuals Experience in triaging participants to mental health programs offered through benefit plan or community programs Wellness/disease management coaching and program development experience Diabetes education experience Community outreach experience Extensive knowledge of, and application of, the social determinants of health Familiarity in data analysis (utilization/program outcomes) and reporting
