Health Care Navigators engage in proactive outreach to build successful working relationships between members and healthcare professionals; helping to support, engage, and refer members to available programs, resources, and services to navigate the complex health care system. They work effectively with our healthcare professionals “Care Teams” to empower members and their families towards positive change and participation in their health.
SKILLS
Health Care Navigator
Works collaboratively with Clinical Operations department to provide support, health education, and to members and their families using an empowerment approach to promote positive change and participation in their health. Engages and educates members about available programs, additional resources, and GEHA services. Documents information in appropriate systems.
Duties and Responsibilities
Collaborates with internal teams utilizing appropriate tools to prioritize workload
Conducts follow-up to assist members with reaching goals and closing gaps in care
Facilitates appropriate use of resources. Coordinates necessary services to improve health status for members across teams
Identifies member needs for and refers to appropriate internal and external programs. Redirects members to in-network facilities when available
Provides support, empowerment, education to identified members as designated
Assists members with identifying a preferred health care provider using appropriate GEHA resources
Accurately documents work with members in appropriate systems following policies and procedures
Identifies complex care issues and refers to the appropriate department
Education Requirements
Requires two years (60 credit hours) of college.
Additional years of qualifying work experience may be considered in lieu of formal education
Experience Requirements
Ability to work as part of a multidisciplinary team
Requires effective written and verbal communication skills
3 years’ experience in a clinical setting, human services, care management or healthcare call center
Must reside in the Kansas City metro area
Must be able to work 9am to 6pm Monday through Friday

The annual base salary range for this position is $42,853 - $60,407 USD.
