Family Support/Resource Specialist Part Time
JOB SUMMARY: The Healthy Families Monroe Family Support/Resource Specialist isa dually trained role, responsible for providing direct service home visiting support tofamilies along with completing Intake Assessments (Parent Survey's) which may includeinitial assessment and intake, case management, parenting skills training, communityadvocacy, information, referral and concrete services. All casework services aredocumented in the HFNY recording system (MIS).
Essential Functions:Conduct interviews of parents referred to the program to assess their needs.Provide home-based, direct services to qualified parents and childrenConduct referral and case coordination, crisis intervention, and assistance in securing tangibleneeds, as necessary.Diligently manage a caseload required by the HFNY contract.Refers children/parents to appropriate community resources as necessary.Ensure all records of referrals and open cases are accurate and up-to-date per programrequirements.Maintain working relationships with community agencies that can be utilized as resources forpersons referred to the program.Actively participate in all levels of individual and group supervision.Attend and engage in mandatory trainings and community meetings.Complete required case notes and activity within the Healthy Families New York system (MIS).Complete all related administrative tasks which includes the maintenance of all necessary recordsand reports.Accompany and/or transport clients to services, when applicable.Completes other duties as assigned by the Program Supervisor and Manager.Exemplifies a demonstrated commitment to diversity, equity and inclusion work and deliveringservices within a trauma-informed and equitable lens.QUALIFICATIONS:Minimum of a High School diploma with at least 3-5 years' experience in the human servicesfield; Bachelors Degree preferred.Experience working with families a plus.
