JOB REQUIREMENTS: Pillar Health is looking for a full time, experiencedFamily Practice Nurse Practitioner. POSITION SUMMARY Nurse Practitionersmanage health problems and coordinate health care for the patient inaccordance with State and Federal rules and regulations and the nursingstandards of care (assessment of health status, diagnosis, developmentof plan of care and treatment, implementation of treatment plan, andevaluation of patient status). ESSENTIAL FUNCTIONS The list of dutiesand responsibilities is not all inclusive and may be expanded to includeother duties and responsibilities, as management may deem necessary fromtime to time. Functions independently to provide preventative, acute andchronic care to patients within scope of certification (family, adultetc.) * Reviews patient file/record, including allergies, problems,medications, and immunization status. * Elicits and records informationabout patient\'s medical history. * Examines patients for symptoms orphysical information. * Orders or executes various tests, analyses, anddiagnostic images to provide information on patient\'s condition. *Analyzes reports and findings of tests and examination, and diagnosescondition of patient. * Administers or prescribes treatments. *Determines and prescribes medication, dosage, and schedule given thepatient\'s condition and allergies. * Discusses any possible sideeffects and alternative treatment options to medication or immunizationwith patient. * Prescribes vaccinations to immunize patient fromcommunicable diseases. * Promotes health by advising patients aboutdiet, exercise, hygiene, and methods for prevention of disease,including STDs. * Documents the patient\'s visit including medicalhistory, physical exam, diagnoses, and plan of action. * Work in acollaborative relationship with team members and physician to managecomplex cases * Other related duties as required POSITION REQUIREMENTSEducation * Degree from an accredited nurse practitioner program *Current Advanced Practice Nurse license in the State of Wisconsin *Current certification in specialty area * BLS certified WORK SCHEDULEOpen availability to work a minimum of 40 hours during clinic operatinghours, typically Monday - Thursday, 7am - 7pm, Friday, 7am - 5pm androtating Saturdays 7am - 3pm.
