Full-Time Family Health Worker - Dental - Manhattan

Responsibilities :
Manage and track patient referrals
Facilitate urgent referral for patients
Direct patients to appropriate specialist referral
Assist in obtaining consultation/specialists reports back to the Health Center
Ensure that the patients/families have adequate understanding of referral procedures and the reason for specialist referrals
Provide education and support to patients/families about the importance of keeping the specialist referral appointments
Assist staff with orientation and training of volunteers and trainees
Assist patients with scheduling appointments with specialists
Collaborate with social workers to provide assistance and support to patient with financial or health insurance issues
Act as the point of contact for patients/families with issues related to referrals
Generate weekly referral and test tracking EDR reports
Follow up with patients post-specialist visits
Perform other duties as assigned
Minimum Qualifications :
Two years community college or equivalent
One to two years community health or health care experience
Bilingual in English and Chinese (Cantonese and Mandarin)
Compensation and Benefits :
Salary: $40,000.00 - 45,000.00 per year
