Description:
Saint Francis Hospital & Medical Center, Hartford, CT
You would be responsible for coordinating and implementing the diabetes education program with the assistance of other team members. Serves as a resource person for patients, families and health practitioners on issues relating to diabetes.
Provide direct patient care and counseling to all ages of patients, particularly the adult patient.
What you will do:
Coordinates various curriculum and programmatic aspects of the Diabetes Care Center with emphasis on the process of Recognition by the American Diabetes Association.
Conducts nursing assessment to establish patient's physical, psychosocial and educational needs to all ages of patients, particularly the adult patient.
Plans and provides for didactic and individualized patient education and evaluates effectiveness of nursing intervention.
Implements discharge regimen.
At discretion of the Director, in collaboration with other team members, may assume responsibility for the management, evaluation and overview of the diabetes self-management education program.

40 hours, Day ShiftMinimum Requirements:
Bachelor’s degree in health-related field.
2-3 years, prior clinical experience. Experience in ambulatory services/ chronic disease management preferred.
Registered Nurse (RN) with current license in Connecticut, pursues and maintains certification as a Diabetes Educator (CDE); or Registered Dietitian (RD) with a CDE.
Ability to perform individual and group teaching. Ability to assist with program planning, management and evaluation. Ability to assess patient needs and to develop/evaluate educational objectives.
