Job SummaryThis position is for a case manager with the Access Transformative OutreachProgram (ATOP). ATOP is a Colorado Access-funded program at Denver HealthOutpatient Behavioral Health Services (OBHS). ATOP supports folks who havea primary substance use disorder diagnosis and would likely benefit from carethat is comprised of a psychiatrist, nurse, clinician, case managers,care navigator and peer recovery coach. We provide assertive multidisciplinarycare that is community-based, meeting clients where they're at both in thecommunity setting and their stage of change.
Masters level therapist, under supervision of a licensed therapist,performs culturally competent individual, family and group therapy, careplan development, case management, family intervention, and referralwork within Colorado BHA Standards for patients receiving psychiatric andsubstance use disorder treatment at Denver Health. Subscribes to anevidence-based practice framework to manage and facilitate clinical processesand progress for patients. Effectively manages a clinical caseload andclinical documentation that spans multiple modalities of care. Goals ofclinical practice are to assist patients in their ability to effectivelymanage/improve their psychiatric and substance use disorders, to functionbetter with the community and gain stability within the community setting.This position does have an office home-base at Denver Health, but is primarily
Essential Functions:Psychotherapy: Provides individual, family and/or group psychotherapy toaddress identified mental health and/or substance disorders (under thesupervision of a licensed therapist on the team). Documents therapytreatment plan, interventions, and client response to treatment accordingto State guidelines, ensuring that clinical supervision records are current.(9Patient Assessment: Identifies patients' conditions, problems, anddetermines diagnosis. Evaluates emotional components, impacts of healthissues and assists client to identify strengths, dysfunctional factors andto establish detailed and coordinated treatment plans. Interviews clients toobtain health history and/or complete intake evaluation which may includenecessary paperwork. (10%)
Education:Master's Degree Counseling profession (social work, counseling) Required
Work Experience:1-3 years Experience working in mental health and/or substance use disordersRequired and1-3 years Experience with client assessment and providing individual/familypsychotherapy as well as group therapy and documenting clinical progressthroughout the treatment episode. RequiredCase management experience HIGHLY preferredExperience working with unhoused and/or substance abuse population
Knowledge, Skills and Abilities:Demonstrates knowledge and applicability of the principle of growth anddevelopment over the lifespan, as well as demonstrating the ability toassess data reflecting the patient's status and interpreting appropriatecultural information of the patient(s) to whom care/service is beingdelivered/provided.Knowledge of evidence-based clinical framework.Knowledge of the disease progression of mental health or addictive disorders.Knowledge of medical terms, abbreviations, diagnosis, treatment andprognosis, and demonstrates this knowledge verbally and in writing.Ability to motivate persons for positive change.Clinical thinking skills that lead to the identification of the strengths andweaknesses of alternative solutions, conclusions or approaches to problems.Ability to provide linical assistance, emotional support and skillsbuilding, or other necessary clinical care to patients when needed.Ability to maintain professional boundaries and confidentiality of recordsrelating to clients' treatment.Ability to develop effective therapeutic interventions that aresocial-culturally sensitive.Proficient in Microsoft Excel, Word, PowerPoint.
