The role of this position(s) is to: Chore: provide the above day to dayhousehold maintenance tasks that are needed to maintain a consumer?shome in a clean, sanitary and safe manner. This service includes heavyhousehold chores such as washing floors, windows and walls, securingloose rugs or tiles, moving furniture to provide safe access and egress.Homemaker: provide housekeeping, shopping, laundry, essential errands,basic meal preparation and meal planning (not for special diets),maintain a safe environment, observing and monitoring and reporting onconsumer condition and teaching those tasks. Companion: providenon-medical care, supervision and socialization to a consumer. Assistand/or supervise a consumer with light household tasks such as mealpreparation, laundry and shopping. Provide services designed to develop,maintain and/or maximize the participant?s independent living skills, toachieve objective of improved health and welfare to support the abilityof the participant to establish and maintain a residence in thecommunity. Individual Support and Community Habilitation: Providesservices which may include locating appropriate housing, theacquisition, retention or improvement of skills related to personalfinance, health, shopping, and use of community resources, communitysafety, and other social and adaptive skills to live in the community.Individual support and community habilitation provide supports necessaryfor the individual to establish, live in and maintain a household oftheir choosing in the community. It may include training and educationin self-determination and self-advocacy to enable the individual toacquire skills to exercise control and responsibility over the servicesand supports they receive, and to become more independent, integrated,and productive in their communities. Personal Care Worker: This positionis to provide a range of personal care assistance that is appropriateand necessary for the participant?s health and well-being. To enableparticipant to accomplish fundamental activities of daily livingincluding but not limited to eating, toileting, dressing, bathing,transferring and ambulation. QUALIFICATIONS: A High School Diploma/GEDor life/work experience with providing care to individuals withdisabilities or the elderly. Ability to complete all initial and ongoingbackground checks including but not limited to: CORI, SORI,Fingerprinting, LEIE, Driving record, E-verify, Physical, PPD andlicensing and/or certification checks (i.e. nursing). Valid US driver?slicense and ready access to a vehicle. TRAINING REQUIREMENTS: Mustmaintain all certification needed in the program including: CPR, FirstAid, and BFAIR Human Rights Training. May need to attend workshops,seminars or BFAIR functions as needed for job development.RESPONSIBILITIES: 1. Treat each consumer, co-worker, supervisor andothers with the respect and dignity due every human being, and adhere tothe Human Rights Standards and the Elder Abuse Reporting Guidelinesthrough the Executive Office of Elder Affairs (EOEA) around physical andemotional abuse, including the timely reporting of violations. Inaddition, will abide by the Disabled Persons Protection Commission?s(DPPC) Mandated Reporter Guidelines. 2. Set appropriate limits withconsumers and problem solve independently. 3. Must exercise goodjudgment, including the appropriate handling of emergency situations. 4.Must communicate effectively, both verbally and in writing, withconsumers, families, co- workers and supervisor. 5.
