Position Summary: Provide health education/presentations, individual health screening/ counseling, group discussion and referrals to other professional disciplines. The Community Nurse will be an integral part of this ISC Health and Wellness team and provide the following types of service to all senior center participants.
Primary Responsibilities:
Perform screenings of senior center members based on the Community Health Assessment tool of the NYC Department of Health and Mental Hygiene;Screen for signs of dementia and memory loss; Provide education on and promote vaccinations; Make referrals to other health care providers in the community as needed;Empower senior center members by providing information they can use when they speak to their health care providers (e.g. a "question to ask your doctor" pamphlet), and act as an advocate for clients;Conduct individual health counseling to clients, esp. those who have low educational background and low/no income.Perform follow-up to ensure that members in-fact followed recommendations;Conduct regular lectures to raise the awareness level of the senior center members about major health issues; Coordinate with the program coordinator to assist evidence-based programs, including chronic disease self-management, diabetes self-management, heart disease self-management, falls prevention, etc.;Coach and monitor the twice-a-week blood pressure screenings conducted by DFTA trained volunteers;Act as a health care resource to senior center members, management and staff as needed; Collaborate with community physicians and other health care providers to promote general wellness among the senior center members;Act as a liaison with area hospital to ensure appropriate discharge planning; &Document case notes in DFTA PeerPlace Database on a timely basis.Supports organization's mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationship with co-workersAdheres to the organization's policy in regards to absenteeism and appearance
Job Competencies & Minimum Qualifications:
Current license and registration to practice as a Registered Nurse in New York State. Maintains professional licensure/certification. Graduate of an accredited school of nursing and must be a registered professional nurse with:
(a) a baccalaureate degree in a health or human services field; or
(b) two years' experience in home care.
Salary Range: $48-$50/hour; commensurate with experience
Working Conditions/Physical Demand:
While performing the duties of this job, the employee may be required to sit, stoop, kneel, crouch, crawl, and climb stairs. The employee may occasionally lift and/or move up to 50 pounds and frequently stand, walk, and reach with hands and arms. The employee is required to travel in increment weather (snow and rain).
