  Job Summary

  The Clinical Pharmacist ("Pharmacist") under general supervision,supplies medications and other pharmaceuticals needed for adequate patientcare according to professional standards and regulatory requirements. Educatespatients and healthcare workers relative to drugs and medications usage,adverse drug reactions, side effects, contraindications, etc. accordingto professional standards.Manages the day-to-day activities of the pharmacy support staff.

  Essential Functions:

  Interprets and clarifies orders for all types of medications.Researches and responds promptly to drug information requests.Monitors drug therapies for appropriateness and formulary adherence.Provides therapeutic drug monitoring of targeted medications as applicable.Monitors drug therapy for potential drug-drug, drug-food, and drug-diseaseinteractions.Advises patients, family and medical staff on use of various medicationsincluding dosage, side effects, drug interactions and composition.Directly supervises the work of pharmacy technicians and support staff asrequired by the Colorado Board of Pharmacy.(35%)Prepares or oversees the compounding, labeling and packaging of sterile ornon-sterile medications, products and pharmaceuticals in accordance with allregulatory requirements as applicable.Acts as a consultant on medication-related issues and assisting incoordinating the drug utilization programs.Assists in the development and maintenance of policies & procedures whenwarranted.Maintains secure storage and records of all medications in accordance withmanufacturer, state and federal requirements.Complies with established departmental and DHHA policies and procedures,quality improvement, and safety initiatives.Completes and meets the competency standards including education and trainingthat is specific to the patient populations and practice site served. Keepsjob knowledge current.Assists in the training of pharmacists, pharmacy residents, pharmacyinterns, pharmacy students, and pharmacy technicians.Attends and participates in departmental meetings, in-services, huddles,and departmental trainings as applicable.(35%)Understands patient safety goals and other quality metrics that affect area ofpractice.Reports and follows up on medication or patient safety issues as needed;works to resolve problems in a manner that will improve patient safety andteam efficiency.Reports medication events (adverse drug reactions and medication errors)using the organizations' event reporting system.Collaborates with key stakeholders across the health system regarding patientcare and projects.Adheres to and complies with and demonstrates support for the mission andvalues of Denver Health.Adheres to Denver Health and departmental standards of behavior and attendanceguidelines.(30%)Education:

  Doctorate Degree PharmD (preferred) or BPharm RequiredWork Experience:

  1-3 years PGY1 residency completion preferredLicenses:

  PHA-Licensed Pharmacist - DORA - Department of Regulatory Agencies RequiredACLS-Advanced Cardiac Life Support - AHA - American Heart AssociationBLS-Basic Life Support (BLS/CPR) - AHA - American Heart AssociationBCPS-Board Certified Pharmacotherapy Specialist - BPS - Board of PharmacySpecialtiesPALS-Pediatric Advanced Life Support - AHA - American Heart AssociationPBID-Pharmacy Based Immunization Delivery - BPS - Board of PharmacySpecialtiesKnowledge, Skills and Abilities:

  Strong verbal and written communication skills, ability to interact withother members of the health are team and with pharmacy leadership.Strong organizational skills.

  Thorough knowledge of the principles and practices of pharmacy.

  Thorough knowledge of pharmacy related state and federal laws.

  Ability to maintain accurate records and prepare reports.
