Our Care Managers are frontline advocates for members who cannot advocate for themselves. They are responsible for assessing, planning, implementing, and coordinating all case management activities with members to evaluate the medical needs of the member to facilitate the member's overall wellness. You can have life-changing impact on our members who are enrolled in Medicare and Medicaid and present with a wide range of complex health and social challenges. A client of Insight Global Health is looking for an Care Manager (Registered Nurse) for a remote 6-month contract-to-hire position. This individual will carry out the following responsibilities: Developing proactive care plans to address member health issues and enhance future wellness outcomes, using clinical tools and resources to conduct evaluations and questionnaires regarding members' needs and benefits, applying clinical judgement to incorporate strategies that will reduce risk factors and barriers in members' lives, conducting assessments based off information provided from various sources, collaborating with supervisors, interdisciplinary teams and key stakeholders involved in member healthcare plans, etc. This Care Manager will be expected to utilize case management processes in compliance with regulatory and company policies to ensure maximum member engagement. He/she must have an active and unencumbered RN license
Skills and Requirements
Must Haves:
* Minimum 3+ years health related field -- Ideally in a home health, assisted living, long term care planning, psych, virtual/remote care, or payer/insurance care management environment
* Must have active and unrestricted RN license. Must have state license if state of residence is not a compact state
* Proven Care Management experience, supporting patients or duals members stratified as "high" and "medium"
* Ability to determine member's clinical stability and provide clinical guidance, while partnering with interdisciplinary team members to align on patient/member goals
* Ability to develop care plans using resources like member's condition documentation, HRA content, etc.
* ASN or BSN required
