　　In accordance with Care Management, Discharge Planning, and SocialService policies, the employee monitors and coordinates the plan ofcare; identifies and evaluates patients individual needs; and assistspatients, families and health care professionals in addressing theseneeds and in developing a plan for post-hospital care. The employee alsoserves as an advocate for patients and families and as a resource andeducator for medical staff and hospital personnel.
