Description
Humana is seeking a Care Coordinator to join the Wisconsin Medicaid Market (iCare) team. The Care Coordinator provides care coordination services to meet medical, behavioral health and social needs of members while promoting and maintaining the quality of care for members. Join this dedicated team and lead change in how health care for the underserved in Wisconsin is managed.
Responsibilities
Conducts initial and annual assessments of members' medical, behavioral and social needs. Coordinates and organizes information from members' assessment to develop a care plan that addresses their needs and assures implemented interventions are collaborated with members and their providers/supports.Builds constructive working relationships with physicians, social service providers and others by involving them in the Care Coordination process and assists in problem resolution involving members and providers as a member advocate, including information for the member grievance process.Reviews, monitors and updates care plans as indicated.Provides information to members and providers regarding benefits, service providers and access protocols and educates members on appropriate use of medical services. Provides referrals for community resources and social services as necessary.Documents member information and contacts made regarding member's care and services provided.Actively monitors member's ability and willingness to engage in treatment regimes, and problem solves alternatives or solutions to their needs. with staff, members and providers,Conducts follow up calls post hospitalization and emergency room visit according to policy.Educates members on preventive health guidelines and programs.
Required Qualifications
Bachelor's degree in social service, rehabilitation, psychology, other related social or human service degree.Health care or social work experience.Previous experience working with Microsoft Office, including Outlook, Teams, and Excel.
Preferred Qualifications
Previous experience working with persons with disabilities in a health care or social service setting.Experience completing intake or initial assessments.Bilingual English/Spanish or English/Hmong.General knowledge of Medicaid and Medicare benefits.
Additional Information
Workstyle: This is a hybrid home position where employees primarily work from their home office, occasionally using company office space for collaboration and face-to-face needs. #LI-HybridWork Location: must reside in Wisconsin.Travel: may require travel weekly to closest Humana/iCare office location.Typical Work Days/Hours: Monday - Friday, 8:30 am - 5:00 pm CST
