Description
Neighborhood Health Center (NHC) is GROWING and actively seeking a Care Coordinator. The Care Coordinator will have responsibilities that include, but are not limited to, utilizing data analytics to close care gaps in clinical quality measures, using motivational interviewing to improve health outcomes, and supporting the administrative functions for the Quality Department. For more than 30 years, NHC has delivered quality healthcare to the communities we serve.
Responsibilities:
Utilizes data analytic platform to close care gaps in clinical quality measures
Utilizes a patient-centered approach and motivational interviewing to improve health outcomes
Supports administrative tasks of quality department, including running reports, entering survey data, and assist with meeting preparation and organization
Completes population data management utilizing computer programs
Assists patients and families with accessing, navigating and connecting with internal providers and services, outside community resources and programs. Provides education and outreach when appropriate and needed to improve patients’ quality of life, health and wellbeing
Supports in-reaching and incentive yearly plan to connect current and new patients to services within the health center and to other specialty/referral providers
Maintains in-reaching/incentive registries and reports to support the in-reaching framework and plan
Schedules appointments accurately in accordance to Neighborhood Health Center (NHC) policy, if necessary
Completes, tracks, and reports metrics and outcomes of in-reaching/incentive efforts
Able to problem-solve independently to assist patients and providers in completing care plan goals
Advocates for patients when interacting with community agencies and services
Consults with providers regarding patient needs and making appropriate referrals or linkages
Assists patients in accessing and completing referrals as ordered by providers; obtains prior authorizations, as needed. Documents and tracks referrals until completion
Assists patients in accessing health insurance by connecting patients to appropriate facilitated enrollers
Works as a team member along with nursing, provider, CHW, billing, and other NHC staff to ensure that the patient’s needs have been addressed
Attends in-services and training or other required meeting to enhance professional growth and development through education programs
Assists in the development of procedures and policies related to accountabilities
Assists in developing and implementing metrics to measure effectiveness of this position
Responsible for complete and accurate documentation of projects and assignments
Other duties as assigned
Hourly Rate: $15-$17 an hour (based on a full-time, 40 hour work week)
Individual compensation is based on various factors unique to each candidate, including skill set, experience, qualifications, and other position related components.
Requirements:
High School Diploma or equivalent required. Associate’s degree, preferred
Experience in Community Health, Patient Resource Coordination or equivalent
Computer skills, clerical skills, data entry and organization required
Must have excellent written and verbal communication
Must be able to read, write and speak the English language; a second language of Spanish preferred
