Summary This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. Responsibilities Patient navigation in cancer care refers to specialized assistance for the community, patients, families, and caregivers to assist in overcoming barriers to receiving care and facilitating timely access to clinical services and resources. Navigation processes encompass pre-diagnosis through all phases of the cancer experience. Breast Care Nurse Navigator (BCNN) Case Management is clinically assigned to assist in the Women Veterans Program (WVP) providing care for Veterans which encompasses various tasks navigators provide across the cancer care continuum screening, diagnosis, staging, and treatment which include chemotherapy, radiation, mastectomy, then survivorship, and end-of-life). Establish an effective relationship with the Veterans and significant others to facilitate the development of the care. Using the nursing process to develop and implement a plan of care: collect appropriate assessment data for each client, determine appropriate nursing diagnosis, identify expected outcomes, determine the plan of care, implement interventions, and evaluate client outcomes. Develop assessment and management strategies based on a consideration of the physical, functional, cultural, social, economic, developmental, and spiritual dimensions of human responses to actual and potential problems. Demonstrate knowledge of the impact of actual or potential illness on the patient's developmental, physical, social, emotional, age, spiritual, economic, vocational, and leisure status. Coordinate and collaborate with patients, families, communities, and members of the interdisciplinary team in assessing needs, setting goals, planning interventions, providing care, and evaluating outcomes. Apply the existing body of scientific knowledge in health care to critical care nursing practice. Participate with patients, families, and other health care providers in collaborative decision making that reflects the understanding that care should be culturally sensitive, ethical, legal, informed, compassionate and humane, and within the boundaries of available economic resources. Educate patients and significant others about measures that promote, maintain, restore health and promote comfort. Completing all required training, leading and executing process-level improvement projects, collecting process data, applying problem-solving and quantifiable tools to an improvement project, and eliminating waste and defects as requested. The Women Veterans Program BCNN participates with patients, families, and other healthcare providers in collaborative decision-making that reflects the understanding that care should be culturally sensitive, ethical, legal, informed, compassionate, and humane, and within the boundaries of available economic resources. Assist patients in understanding their diagnosis, treatment options, and the resources available, including educating eligible patients about appropriate clinical research studies and technologies. Serve as an essential link between patients and all other care providers. Facilitate appointments for consults and support services within established service standards, including social workers, dietitians, physicians, etc. Coordinate multidisciplinary planning conferences, develop concise patient summaries for use by the care team, and document recommendations made utilizing standardized care protocols in accordance with nationally recognized care guidelines. Develop patient education programs and tools. Follow patients throughout the care continuum, including inpatient admissions, and collaborate with inpatient care management resources. Work with Cancer Registry staff and others to collect data, track outcomes, and support strategic planning processes.
