Job SummaryUnder minimum supervision, performs one-on-one patient centered needsassessment, care coordination, and behavioral health education to patientsand other parties that may be involved in a patient's care, such asfriends/family members and other professionals. The Behavioral HealthEducator will conduct telephonic screening of patients for substance usedisorders, depression, anxiety, and overall psychosocial functioning,and provide referrals and guidance on accessing appropriate internal andcommunity resources. Other duties include collaborating with hospital widehealth education initiatives as needed, identifying new and existingresources and organizations that serve patient health needs, and maintainingcommunity partnerships. This position will perform the above duties primarilytelephonically and will be responsible for attending the Denver HealthSubstance Treatment Line - a phone line available to individuals andprofessionals within the Denver community to call with questions or to receivesupport accessing treatment for substance use disorders.
Essential Functions:
Provide support and resources to individuals and families who are impacted bybehavioral health issues 30%Develop and distribute educational materials, including brochures,handouts, and online resources 20%Collaborate with interdisciplinary teams to develop and implement behavioralhealth education initiatives 10%Develop and deliver educational programs on a variety of behavioral healthtopics to individuals, families, and groups 10%Maintain accurate and timely documentation of training and educationactivities 10%Participate in quality improvement initiatives and peer review processes 10%Participate in community outreach activities to raise awareness of behavioralhealth issues 5%Conduct assessments to identify training needs and develop training plans 5%
Education:Bachelor's Degree in public health, human services, health sciences,psychology, sociology or related field required.
Work Experience:Three years health care or health education experience required.1-2 years conducting behavior change and/or health education required.
Knowledge, Skills and Abilities:Knowledge of comprehensive health education and substance use, andexperience with behavior change counseling.Ability to coordinate health education within the clinic and also withcommunity resources.Proficiency in MS Office applications; experience with Excel, Word, andPowerPoint.Experience facilitating one-on-one confidential visits and managing caseloads.Demonstrated ability as a self-starter who can prioritize and conduct multipleactivities.Strong communication and interpersonal skills; a team player.Exceptional organizational skills, excellent oral and written communicationskills and creative problem solving abilities.
