JOB REQUIREMENTS: Overview The Behavioral Health Case Manager works withthe inpatient behavioral health/medicine multidisciplinary team toprovide care for the patients admitted to those units. The BHCM wouldhelp with dispositional planning, care planning and group therapy. TheBHCM also helps with behavioral health consults in the ED and on themedical unit. Under the supervision and consultation of a psychiatristthe BHCM will gather data about patient condition and assist withdisposition, pro viding resources and recommendations to patients in theED or on medical floors. Performs other duties as assigned.Responsibilities Works with patients on the inpatient unit, emergencyroom and medical floors. Consults and coordinates care of patient withthe physician/psychiatrist, inpatient RN and other agencies. Assessestreatment planning and progress. Prepares treatment plan and goals forpatients. Maintains pertinent patient data in the EMR in accordance withstate and federal laws and hospital policy. Provides group therapy.Arranges for disposition and follow up recommendations. Education andExperience Master\'s Degree in a mental health/behavioral health fieldwith documented clinical course work. Experience or field studyexperience in mental health site. Certification/Licensure TACT certifiedwithin 90 days after hire.
