The scopeof responsibilities includes collaborating and coordinating withphysicians and other health professionals regarding clinical practice,education, consultation, research and program development. SpecializedResponsibilities: (This may not include all duties assigned.) *Performs comprehensive health assessments for acute and chronicpatients. * Assesses and manages care for acute, minor, episodicillnesses and injuries. * Interviews and advises patient/familiesregarding health/illness prevention. * Performs follow up exams. *Provides assessment, evaluation, and management of clients with acuteand chronic illnesses. * Collaborates with patients to determinephysiologic, emotional, and environmental needs and priorities. *Provides case management services to assist clients and families withthe arrangement for, or refer patients to needed services. * Managespatient health concerns through telephone triage and consultation asneeded. * Provides services, which may include the ordering ofappropriate diagnostic and laboratory tests, the prescription ofpharmacological agents, and other non-pharmacological treatmentmodalities. * Maintains and reviews patient records, charts, and otherpertinent information. * Performs patient callbacks, reports diagnosticfindings and provides consultation. * Participates with physicians inreview of patient\'s health records. * Participates in call coveragewhen required. * Attends required department meetings and participatesin committees as requested. * Other appropriate clinical duties asassigned by supervisory physician(s) in accordance with experience. *Promotes ongoing quality improvement activities. * Documents in theelectronic medical record for all patients.
