Position Summary
The Health Coach Dietitian role is fully remote and candidates can apply from any state, however, there is a preference for candidates that live in either MST or PST time zones.
Normal Working Schedule: Monday -Friday 8:30am-5:00pm in time zone of residence with flexibility needed for member calls after 5pm.
Utilizes a collaborative process of assessment, planning, implementation, and evaluation, to engage, educate, and promote/influence members in decisions related to achieving and maintaining optimal nutritional status.
Assessment of Members:
-Through the use of clinical tools and information/data review, uses knowledge and experience of nutrition and diet planning to conduct comprehensive evaluation of member’s nutritional status and risks.
-Utilizes assessment techniques to determine member’s level of health literacy, technology capabilities/access, and readiness to change.
Enhancement of Medical Appropriateness and Quality of Care:
Assimilate health/nutritional assessment data to establish a recommended nutritional approach based on member’s health status/risks, education, preferences, and support needs
Partners with members to identify opportunities for integration into available internal/external programs.
Using holistic approach consults with supervisors or others to overcome barriers with assisting members in meeting goals and objectives.
Identifies and escalates quality of care issues through established channels.
Utilizes influencing/motivational interviewing skills to ensure maximum member engagement and promote lifestyle/behavior changes to achieve optimum level of health.
Utilizing interviewing and active listening skills, provides nutritional coaching, information, and support to empower the member to make ongoing independent healthy lifestyle choices.
Interprets and utilizes clinical guidelines/criteria to positively impact members health.
Provides up-to-date information to help facilitate the member’s understanding of his/her health status.
Helps member actively and knowledgably participate with their provider in healthcare decision-making.
Required Qualifications
Registered Dietitian licensure through the Commission on Dietetic Registration
Registered Dietitian Licensure if required in the state of residence
Upon hire, it will be required to obtain licensure in multiple states (approximately 20 states) where members reside, with CVS supporting the cost of required licensure
5+ years of experience working in a multidisciplinary team
5+ years of experience with weight management, disease management, medical nutrition therapy and health lifestyle behavior change
5+ years of coaching experience
Requires strong technical skills for working with multiple monitors and programs, proficiency with MS Office Suites (Outlook, Word, Excel)
Must be willing to work Monday -Friday 8:30am-5:00pm in time zone of residence with flexibility needed for member calls after 5pm. Candidates in EST time zone may need to work at least from 9:30am-6:00pm EST as their standard shift to provide the needed coverage for members in MST and PST time zones
Preferred Qualifications
Certified Diabetes Care and Education Specialist (CDCES)
Candidates from MST/PST time zone
Experience in facilitating patient/client positive behavioral change
Background in motivational interviewing, active listening skills, Trans theoretical Model, mindfulness based, and other evidence-based coaching techniques preferred.
Managed Care Organization experience preferred
Demonstrated ability to communicate, problem solve, and work effectively with people, use good judgment in challenging situations
Education Bachelor’s degree required
Pay Range
The typical pay range for this role is:
$46,597.20 - $101,200.00
This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
