Community Health Worker?Bilingual Spanish Community Health Worker (CHW) will bridge the gap between immigrant, refugee, and Muslim communities and health/social services systems. This includes walking alongside community members as they navigate complex systems to assure community members are receiving services. This Bilingual CHW will continue building strong relationships with partner agencies and other CHWs. The new Bilingual CHW will engage and support community members as well as SDVFPs clients who are currently in the immigration legal system. This position will collaborate with all teams within SDVFP.
ESSENTIAL JOB FUNCTIONS
Network with other community partners and healthcare providers to understand services available and to build trust with partner organizations and SDVFP.Use Bilingual skills to refer, support, and connect clients and families to resources including food, housing, health insurance, health information, quality healthcare, and other social services.Coordinate transportation to appointments and serve as a client/patient advocate and interpreter when needed.Bring awareness to community partners and institutions regarding barriers to services for multilingual community members and advocate for access.Collect Data and document referrals and interactions with clients and community members using the system developed by SDVFP.Attend community events including events with other SDVFP teams for visibility as a community resource and educate on programs SDVFP offers in the community.Travel as Required.Troubleshoot barriers to access to healthcare and social services.Identify and understand individual and community needs.Serve as liaison between families, neighborhood centers, and schools to understand the fundamental needs of the community when they are in crisis.Maintain client confidentiality at all times.```{=html}
```- Attend meetings, training, and seminars as deemed appropriate and necessary.- Proficient in Microsoft Office, Google Suite, WhatsApp, and Case Management systems.- Enhance the clients ability to effectively communicate with healthcare and service providers while understanding their needs and community resources available to them.
QUALIFICATIONS
Ability to communicate effectively both orally and in writing.Proficient in speaking, reading and writing in both Spanish and English.Ability to complete CHW Certification within a year.Ability to maintain flexible hours to meet the needs of clients and community events.Ability to work respectfully and effectively in teams, across SDVFP program teams, work in community and work independently to obtain identified goals.Ability to identify barriers to access to social services and provide critical thinking and creative solutions.Ability to multitask and prioritize work assignments and client needs.Ability to use trauma-informed approaches to care and conversation.Knowledge and familiarity with culture and community of immigrant/refugee/undocumented population(s) in SD and immigration issues impacting these communities.Demonstrated commitment to the mission and purpose of SD Voices for Peace including the core values of service, social justice, dignity and worth of the person, importance of human relationship, integrity, and competence.High school diploma, GED or 5 years of demonstrated experience in community work.Demonstrated commitment to community development with preferred experience working with multilingual communities and vulnerable clients.TO APPLY
