　　This is a Clinical Pharmacist Position supporting Medicaid Prior Authorization.

　　The position is remote within the state of Oklahoma, but you MUST RESIDE WITHIN THE STATE OF OKLAHOMA.

　　This position requires you to obtain AZ and TX pharmacist licensure within 90 days of hire.

　　We are hiring for various shifts (8 hours daily), between 7am - 8:30pm CST.

　　The training schedule will (tentatively) be: 9:00 am - 5:30 pm CST for 8 weeks

　　You are required to work one weekend every 6 weeks and occasional holidays.

　　Required Qualifications

　　MUST RESIDE IN OKLAHOMA

　　Obtain active and unrestricted AZ or TX pharmacist license within 90 days of hire

　　3+ years experience working as a clinical pharmacist.

　　Experience with clinical decision-making while applying evidence based clinical guidelines/criteria; utilizing formularies/preferred drug lists.

　　Experience working in a high production team environment

　　Demonstrated experience with clinical program components, formulary designs, and prescription benefit trends.

　　Preferred Qualifications

　　Medicaid or Medicare experience.

　　Experience reviewing Medical drugs (Jcodes and Qcodes) is a plus

　　Prior authorization experience

　　Experience reviewing medical records, clinical literature, and composing SOAP notes.

　　Education

　　B.S Pharm or PharmD from an accredited school of pharmacy required.

　　Must be licensed Pharmacist in good standing

　　Pay Range

　　The typical pay range for this role is:

　　$76,050.00 - $163,800.00

　　This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
