Responsibilities*
Duties
Support clinical triage and new referrals to facilitate appropriate scheduling, patient needs and timely access to care in collaboration with the intake coordinators.
Integral member of the multidisciplinary team including tumor board discussions and follow up.
Simplify complex medical information into language patients, families, and caregivers can understand and apply.
Communicate with patients, families, and caregivers at identified touchpoints in their care (i.e.: new patient appointment, discharge from hospital, chemotherapy follow up and transitions in care)
Monitors the patient, family, and caregiver understanding and compliance with the plan of care.
Assesses and provides referrals for patients who have tangible barriers to care such as finances, transportation needs and prescription costs.
Participates in the tracking and monitoring of metrics and outcomes, in collaboration with clinical administration to document and evaluate program outcomes.
Serve as an essential liaison between patients and all other care providers.
Collaborates with lay navigators to facilitate appointments for consults and support services within established service standards including social workers, dieticians, physicians etc.
Develop patient education programs and tools.
Responsible for outreach efforts to establish and maintain positive working relationships with key members of the healthcare team and community resource groups.

Utilizes the nursing process and nursing standards to prevent or manage actual/potential patient problems related to their disease and /or treatment.
Identifies patient /family learning needs and ensures that they are met.
Serves as a patient advocate. Assists with care coordination between multiple departments.
Works with the various Cancer Center associates to ensure patients are scheduled appropriately.
Able to prioritize urgent medical issues, clinic flow, administrative tasks.

Inputs, retrieves, and interprets data from computer-based information systems.
Competent with telephone triage related to patient calls.

This UMPNC RN posting is posted under the Role-Specific Advancement Model (RSAM) as:
ENC- COMPETENT NE
Actual RSAM LEVEL and salary will be determined at time of hire.
RSAM levels range from ENC-COMPETENT-NE to ENC-EXPERT-NE TO ENC-MASTERY-NE.
Required qualifications must be met by the candidate in order to be interviewed and considered for the position.
Posting may be filled after the initial 5-day posting period.

Required Qualifications*
A current license in the State of Michigan as a Registered Nurse.
Required Educational Requirement; applicant must meet one of the following: Bachelors degree in Nursing OR an Associates degree or diploma in Nursing and a Masters degree in Nursing
Eighteen (18) months of recent Radiation Oncology RN experience
Two (2) or more years of recent (within the last five (5) years) Hematology/Oncology RN experience

Desired Qualifications*
Experience as a Nurse Navigator
Recent experience in an ambulatory care clinic setting within the last five (5) years as a RN; to include skills in telephone triage and symptom management
Experience and knowledge of the targeted oncology population as described in the posting.
Possess certification by OCN or other related oncology certifications.
Recent experience with chemotherapy management, patient education and symptom management.
Proven leadership ability including: initiative and creativity as evidenced by previous employment reference and past performance evaluations.
Has critical thinking skills to cross cover within clinical areas, coordinate and perform multiple activities and problem solve as measured by interview, references, and performance evaluations.
Demonstrated ability to practice independently, set goals, and promote teamwork.
Demonstrated ability to work collaboratively, efficiently, and communicate well with others (including peers)
Demonstrated abilities in interpersonal, verbal and written communication skills, organizational skills and problem-solving skills within all levels of the health care team
Work Schedule
Hours: 40 hrs/wk
Shift: Days, Monday thru Friday. Hours will vary and flexibility is required.
Location: University of Michigan Rogel Cancer Center, Radiation Oncology Clinic
All new employees are expected to float in times of low census.
