As an agency we are inspired by our Mission of bringing hope to life, especially those most in need.Pay Rate : $21.00 - $26.00Benefits:* Medical/Dental* FSA* 403(b) Retirement* Additional Voluntary Benefits* Paid Holidays* Vacation/Sick Leave* Education Assistance Program* Employee Assistance Program* Longevity awardsPosition Summary:Under the direction of the Crisis Services Program Manager the Certified Peer Counselor provides peer support and occasional case management and other clinical services as needed.Responsibilities:Meets with supervisor weekly for supervision and/or consultationProvides outreach to individuals where they are at in the regional justice system and follow up support upon discharge to connect to longer term servicesProvide outreach and engagement to local area MCOs, BHASOs, regional jail systems, outpatient mental health or substance use disorder facilities, and all providers within the local crisis system of care.Assess each individual from the referral list to determine which services and resources are identified as needed by the individual using a person-centered approach and then help them receive needed services.Support the completion of Quarterly outcomes reports to include:Narrative questions and # of referrals from each local area and referral sources (MCOS, BHASOs and Crisis Stabilization centers)Number of enrollmentsNumber of engaged in Behavioral Health services and number housedProvide short term peer support and if needed case management and related services to or on behalf of the person servedServices will be provided in the community, in a consumers home or in one of the program referral locations, they will provide peer support and collaborative support with the regional justice providers, outpatient mental health and substance use providers, and the local crisis system of care. Support may include:Crisis intervention or services provided as follow up to crisis interventionAssisting with applying for benefitsActivities of daily living, psychological support, service coordination, linkage, transportationAccessing community resources and/or housing/housing retention and connecting to any needed, available, and eligible services up to including medical, mental health, vocational, educational, financial, etc.Assists in referral and provision of:IntakeFamily treatmentMedication managementPeer support, psychological assessmentTherapeutic psychoeducation including Illness Management and Recovery, and referrals to ITA or LRA services.Provides continuity of care in support of established client/therapist relationships and best practices, coordinates care with providers internally and outside of the agency with a Release of Information.Is responsible for maintaining safety precautions and constructive, professional relationships with Medical Services, Substance Use Services, Supportive Housing, Crisis, and Outpatient Team members.Completes appropriate documentation for services provided within required timeframes.Establishes and maintains working relationships with community resources, coordinates consumer treatment with other providers, and initiates referrals to medical providers and other community resources when appropriate.Assists in searching for, finding housing and shelter options, allowing for choice whenever possible.Reports and document
