Twice a year, our patients will return to our clinic for routine follow-up to see Neurology, Cardiology, Pulmonary, PT/OT, Nutrition, Social Work, Complex Care, Advanced Illness Management (AIM), and all associated diagnostic testing. Because our patients require care from a wide array of specialists, our nursing team serves as the glue of the multidisciplinary team. They are responsible for triaging communications (via Mychart, phone calls, faxes performing thorough chart reviews/assessments, high dollar specialty drug prior authorizations, completing medical equipment paperwork, assisting with patient procedures (such as lumbar punctures and muscle biopsies), and performing thorough chart reviews/assessments of patient needs. In the setting of rare disease, the Neuromuscular nursing team works diligently to streamline patient care coordination to achieve best patient outcomes.Emphasizes health promotion and disease prevention as they assist patients and families to move rapidly, uneventfully, safely, and as effectively as possible through their episode of care. Delegates, oversees and provides quality, safe and developmentally appropriate patient care. Practices professional nursing and promotes patient advocacy in accordance with the ANA Code of Ethics for Nurses.
